Problem

When an agent edits several files in one patch (Codex apply_patch, or an ACP tool call with several diffs), the server records one file change item. fileName holds the first path and changes holds all of them. Two summaries that T3 Code gives to agents read only fileName:

  • The history sent to a new provider when you switch providers mid-thread (historicalMessage).
  • The "Main thread activity" block a side chat starts with, and the fork delta summary (summarizeDeltaItem).

So after a patch that changes src/a.ts, src/b.ts and src/new.ts, the receiving agent reads only File change: src/a.ts. It doesn't know the other files changed and may redo or contradict that work. Web already shows "Changed 3 files" for the same item.

Change

Both summaries now list every path from changes, and fall back to fileName when changes is missing. Web's projectedWorkEntry (apps/web/src/session-logic.ts) already uses this fallback. The side chat and fork line goes through the existing compactText cap, the same as the user, assistant and command lines next to it. The handoff history is not compacted, because the handoff budget measures each item whole, as it does command output.

Scope and approval

This is a very small fix for an obvious bug, so there is no prior issue. Both producers (Codex since #22541, and ACP) already record every path, and the clients read them all. These two agent-facing summaries were the only readers that dropped them. The fix is two changed lines plus one assertion in each module's existing test.

Overlap search: I checked the local PR index and GitHub for "multi-file", "file change", "handoff file" and "fileName". #29878 fixes the same pattern in the mobile work log only. #27670 (open, perf) moves the historicalMessage line but still reads fileName, so the two PRs would conflict on that line but don't fix the same thing. #28631 changes the ACP adapter's patch text, not these summaries.

Verification

  • Red on main: AssertionError: expected 'File change: src/a.ts' to equal 'File change: src/a.ts, src/b.ts, src/…' (ContextHandoffBudget.test.ts, "carries command outcomes as attributed activity and excludes reasoning"). Also expected 'You are in a side chat beside the mai…' to include '- File change: src/a.ts, src/b.ts, sr…' (ContextHandoffService.test.ts, side chat context case).
  • Green: cd apps/server && vp test run src/orchestration-v2/ContextHandoffService.test.ts src/orchestration-v2/ContextHandoffBudget.test.ts gives 23 passed.
  • vp run check:changed: format, lint, knip, graphql, typecheck (26 packages) and tests (3 files) pass.
  • Not checked: a real provider switch or side chat end to end. The tests call the two summary functions those flows use directly.

Approvability

Verdict: Approved at 708405f

Macroscope's review found this PR approvable — Handoff history and fork/side-chat summaries now name the paths in multi-file patches rather than only the primary filename. Existing context limits remain unchanged, with no changes to file operations or access controls.

Walkthrough

File-change summaries in context handoff and historical messages now list paths from change entries when available, with a fallback to the file name. Tests cover update, add, and rename-style entries.

Explanation

The title names the main change: handoff context now lists every file in a multi-file patch, not only the first. It uses a conventional commit prefix and matches the changed summaries in the handoff and side-chat code paths.

Full details: Description check

Explanation

The description follows the template. It has Problem, Change, Scope and approval, and Verification sections. The problem and fix are clear. The scope section explains why no prior issue exists and gives overlap checks. Verification lists red and green test runs, the check command results, and what was not checked. The template's trailing agent-attribution line is present.

Verified in a real dev environment

verified unchanged head 708405f709e9e997381035d116f957c570e2d37d against github/main at 61c3794c32b0eeebfac09add43e403618c65d2d7 on linux. installed each checkout with vp i and ran one isolated dev server at a time. used repository playwright with /usr/bin/google-chrome, a real git project added through the web app, and real codex gpt-6.1-sol turns through the local proxy. no mocks, fixtures, or replay provider.

  • asked codex to add src/a.ts, src/b.ts, and src/new.ts in one apply_patch call. both servers recorded all three paths in the same file-change item.
  • started a side chat while that turn was still running. main delivered only src/a.ts; this head delivered all three paths. the receiving codex turn quoted the context without tools.
  • switched the composer to another real codex instance with a separate home directory, forcing a portable context handoff. main again delivered only the first path; this head delivered all three. checked the persisted handoff history and delivery, as well as the receiving turn's answer.
  • a normal follow-up retained all three paths. a subsequent single-file update still delivered only src/a.ts to its side chat.
  • blacksmith: cd apps/server && vp test run src/orchestration-v2/ContextHandoffService.test.ts src/orchestration-v2/ContextHandoffBudget.test.ts: 23 passed. git.cafe reports 31 successful checks, none pending or failing. no source changes or pushes needed.

* fix(web): play completion sounds without delay (pingdotgg#29146)

Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.

Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.

This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.

Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)

* fix(server): keep PR watches active after host failures (pingdotgg#29450)

## Problem

PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.

## Change

Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.

The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.

## Scope and approval

This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.

## Verification

`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.
