fix(server): preserve nested Claude subagent completion across turns - #23561
A nested Claude subagent can remain running after it completes. Reproduce by letting a background parent launch a child while the root is idle, completing that child during a user turn, then letting a sibling completion wake the root: the buffered start replays without its completion.
Change
Keep a live completion notification behind its matching buffered start when the task has not been registered yet. The continuation replays both in order and persists the child as completed. Registered-task handling remains unchanged.
Triaged bug via the automated issue flow. This fixes the reproduced completion-ordering defect in the Claude adapter with one regression test.
Verification
Regression fixture fails at the test-only commit with expected 'running' to equal 'completed', then passes with the fix.
cd apps/server && vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 208 passed on the final head.
Server npx tsc --noEmit: passed. Targeted formatting passed; adapter lint reports the existing unused layer warning, also present on base.
Two isolated code reviewers returned MERGE with no confirmed findings. Final proof audit returned SUFFICIENT.
Real web development server on Linux, Claude Opus 5.5 / CLI 2.1.303: repeated the idle-child / user-turn completion / sibling-wake sequence. Before, the child persisted as running and the UI waited on it. After, it persisted as completed with a completion timestamp and zero running subagents.
Server-only proof: fixture matrix has zero changed render paths. Evidence index includes the ordered runtime events and recording timestamps.
After lifecycle recording: prompt at 00:01, user follow-up at 02:06, continuation complete at 03:47. Screenshots and recording supplement the server proof; message history, timestamps, and restart notices differ, so these are not pixel comparisons. Chromium 154, 1280×800 CSS pixels, DPR 2, light theme, default font scaling, no device emulation. No client rendering code changed.
Implemented by Claude Opus 5.5 through the Claude Code harness; coordinated and reviewed by GPT-6.1-sol through Codex, with an independent Claude Opus 5.5 review.
… running (#16355)
A background subagent launches a foreground nested one while the root is
idle; the nested one finishes during a later user turn, and a sibling's
completion then wakes a continuation. The nested subagent ends running.

…running after a sibling's wake (#16355)
A nested subagent started while the root was idle has its task_started held
for the next wake drain. If it finished during a later user turn, its live
task_notification found no node and was dropped, so the drain replayed the
held start alone and left the subagent running forever. Hold that outcome
behind its start so the drain replays both.

Macroscope's review found this PR approvable — This is a small, isolated Claude orchestration bug fix that preserves nested subagent completion notifications behind their buffered task start so replay can settle them correctly. Existing registered-task handling is unchanged, and the reproduced lifecycle is covered by a regression test.

Walkthrough
The Claude adapter now buffers an early task notification behind a matching buffered start when the task is not registered. A regression test checks that a nested subagent remains completed after a sibling-triggered continuation settles.

Summary
Buffer early notifications and verify completion apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter buffers an early notification when the wake buffer contains the matching task start. The regression test checks the nested subagent’s final status and confirms no background work remains after the continuation settles.
The new test comment introduces a lint error. Remove its issue number before merging; other reported CI errors may still need separate fixes.

Full details: Linked Issues check
Explanation
[#16355] The whole-PR diff (merge base to head) changes one hunk in ClaudeAdapterV2.ts, in the task_notification handler. For a task that is not registered (sessionSubagentsByTaskId lacks task_id), the handler appends the notification to wakeBuffers for the native thread. It does so only when that buffer already holds a task_started with the same task_id, and only when the message is not already present. The buffered start and completion therefore replay in order on the next drain, and the child persists as completed. The registered-task path is unchanged: !appliedSubagentOutcomes.has(message) && isRegistered still calls discardBufferedSubagentStarts and the existing outcome handling. This covers the issue sequence (idle-buffered start, completion during a user turn, sibling wake) and the idle case, where a held start receives its completion on the next drain. The regression test (per the PR summary, +189 lines in ClaudeAdapterV2.test.ts) covers that sequence. The test blob could not be read at this head, so test content rests on the PR summary and the prior assessment. The reported suite result (208 passed) is author-reported and not reproduced here. Acceptance criteria for terminal status, completion timestamp, and Lineage display depend on the persisted completed state. The author reports a real-server before and after run; that run is not independently verified here.

Explanation
The whole-PR diff for ClaudeAdapterV2.ts contains only the task_notification hunk that implements #16355. The incremental diff from the previous reviewed head (92f1042) shows many unrelated-looking edits (for example PortScanner, compaction, runClaudeWorker). Those edits are not PR changes: the whole-PR diff shows none of them, so they come from the base content that the branch picked up after the previous review. The regression test in ClaudeAdapterV2.test.ts supports the bug fix and is in scope. The PR summary lists no client rendering changes, and the only changed paths in the supplied summary are the adapter and its test. The complete changed-path list was not enumerated, so no other path is confirmed or excluded.
