Problem

Yellow and red provider status banners offer no way to recheck the provider. Users must leave the thread and open Settings > Providers to refresh.

Change

Add a Refresh button to provider status banners in web and desktop. It refreshes the displayed provider in the thread's environment, including models, through the existing permission-aware command. The button shows pending state and prevents repeated clicks; failures use the existing command error reporting.

Scope and approval

This is a focused recovery affordance for the existing provider refresh capability. The client RPC permission map now requires orchestration-read permission for provider refresh, matching the existing server requirement. It adds no server behavior, settings, or provider-specific paths. Mobile does not render this banner.

Verification

Before:

After:

  • Changed-file lint and formatting checks passed.
  • Both adjacent test files passed, covering 47 tests.
  • All 19 client command-permission tests passed, including the new provider refresh regression test. Contracts and client-runtime typechecks passed.

Implemented with gpt-6.1-sol through the Codex harness in T3 Code.

Approvability

Verdict: Approved at 51ec4d2

Macroscope's review found this PR approvable — Provider-status banners gain a permission-checked, manual refresh action using the existing provider refresh operation. The change preserves server authorization and product defaults, with no obvious material risk requiring human judgment.

Walkthrough

The active thread’s environment ID now reaches the provider status banner. The banner checks refresh permission and provides an action to request a provider refresh with model refresh enabled. The guarded RPC scope requires orchestration-read permission, which authorization tests cover.

Changes: Provider status refresh

Layer / File(s) Summary
Require orchestration-read permission
packages/contracts/src/clientRpcPermissions.ts, packages/client-runtime/src/state/commandPermissions.test.ts
The client-guarded scope map requires orchestration-read permission for serverRefreshProviders. Tests check authorization for the destination environment and denial when the grant is absent or belongs to another environment.
Pass the active environment ID
apps/web/src/components/ChatView.tsx, apps/web/src/components/chat/ChatBannerLane.tsx
ChatView passes the active thread’s environment ID to ChatBannerLane. The lane requires this ID and passes it to ProviderStatusBanner.
Add the refresh action
apps/web/src/components/chat/ProviderStatusBanner.tsx
The banner checks the environment’s refresh permission and adds a refresh button. The button is disabled when permission is absent or a refresh is in progress. The request enables model refresh.

Explanation

The description covers the problem, change, scope, and verification, including before-and-after screenshots and test results. However, the Scope and approval section does not provide a link to a triaged issue or explicit maintainer approval, and it does not clearly justify why prior approval is unnecessary for this new UI entry point.

the prior-approval rule covers new product behavior, including a new entry point when an existing workflow already serves the need. this PR adds provider refresh to thread banners and identifies Settings > Providers as the existing route, but supplies no maintainer approval. preserve the discoverability/recovery problem, agree on the banner action's direction and scope in Ideas, and link that approval before reconsideration. this is an unexecuted dry-run closure draft.

Minor · Functional Correctness · Quick win

Add the refresh RPC to the client scope map.

A cached provider-status banner can render after the read-scoped config subscription is rejected. Because serverRefreshProviders is absent from CLIENT_GUARDED_RPC_SCOPES, canRefresh is true without AuthOrchestrationReadScope. Clicking Refresh then sends a server-rejected request, while the banner shows no inline error.

Verified in a real dev environment

verified head c5b96cedddd930bb2ac429ecbff135a597c73d53 on this linux boat vm, using the real dev web client, playwright from the repo, /usr/bin/google-chrome, and real codex gpt-6.1-sol turns through the local provider proxy. no mocks or replay data. no source changes were needed.

  • reproduced on github/main at 2b1021ce41 in a separate worktree: a missing codex executable produces the error banner without a refresh action. restoring the executable leaves the banner stale until settings refresh runs.
  • on the exact pr head, created the project and conversation through the app. temporarily moved a task-owned symlink to the real codex executable, refreshed status in settings, then exercised the banner action.
  • while a recheck runs, the button reads "refreshing..." and is disabled. an unsuccessful recheck keeps the error banner and makes refresh available again.
  • restored the executable and clicked banner refresh. the banner cleared, models returned, and a new real gpt-6.1-sol turn replied "recovered codex turn passed."
  • nearby behavior: dismiss still removes the banner; settings refresh still restores the eight-model codex catalog.

captured websocket requests from the actual banner clicks:

13:16:12.712 server.refreshProviders {instanceId:"codex",refreshModels:true,...}
13:16:13.479 server.refreshProviders {instanceId:"codex",refreshModels:true,...}
failed recheck -> banner stays, refresh re-enabled
restored executable -> banner clears, model picker restored
real gpt-6.1-sol turn -> recovered codex turn passed.

the two banner requests correspond to the failed recheck and successful recovery click. both target codex, rather than refreshing every provider.

blacksmith-check ran vp run check:changed HEAD^: format, lint, both knip checks, affected typechecks, 50 tests in four files, and compiler checks for all three changed files passed. the explicit base is the pr parent because the worker uses a shallow source snapshot. existing ci reports 30 successful checks.

main: codex error banner has no refresh action

pr: matching codex error banner includes refresh

real banner recheck, provider recovery, model selection, and codex response

Not verified

native electron shells on macos/windows, native ios/android clients, remote/relay environments, yellow warning banners, and restricted-permission or revoked-session recovery were not exercised. the verification above uses a local environment with standard pairing scopes and a real codex error/recovery path; it does not resolve the separate restricted-permission review concern. uploaded video playback and seeking were checked in the t3 browser player.

Could we change the layout for this so that the refresh button is at the bottom of the status bar? Right now looking off the screenshots I think this could be causing layout shifts.

* fix(web): play completion sounds without delay (pingdotgg#29146)

Completion alerts waited two seconds after the client received a completed thread state. Audio was also fetched and decoded only when an alert needed to play, adding a first-play delay.

Notify immediately when the thread shell reports completion, and preload both sounds when a user gesture unlocks audio. Keep completion deduplication and suppression for existing, archived, and stopped runs.

This intentionally removes the continuation grace for completion alerts. A completed run can now notify before an automatic continuation starts. The sidebar's two-second placement grace and server settlement behavior remain in place.

Validation: `vp run check:changed` passes after merging current main, including 40 affected tests, formatting, lint, unused-code checks, affected typechecks, and React compiler checks. Tests cover immediate completion, duplicate suppression across reconnects, preloaded audio reuse, and retrying failed loads on alerts without retrying on every gesture. Audible timing has not been verified in a live client.

* fix(desktop): prepare macOS x64 bytecode under Rosetta (pingdotgg#29484)

* fix(server): keep PR watches active after host failures (pingdotgg#29450)

## Problem

PR watches stopped after eight failed host reads, so a prolonged network outage prevented later CI and review updates from reaching the thread.

## Change

Keep watches active after provider failures reported as `failed`. Read detail and activity concurrently without allowing an activity failure to cancel or hide a permanent detail failure. Repeated known permanent failures still stop the watch and notify the agent; rate limits still pause reads. Non-rate-limited GitHub HTTP 403 responses and GraphQL `FORBIDDEN` responses now use a distinct `forbidden` reason so access refusals are counted instead of retried indefinitely. GraphQL error types survive the client mapping, after the existing requested-reviewer fallback.

The provider's `failed` category also includes unclassified host and decode errors. Those continue retrying at the existing two-minute cadence until the watch ends or is stopped. A fixed outage deadline would recreate the recovery failure for longer outages.

## Scope and approval

This is a focused fix for an obvious bug in existing PR watch recovery, covered by the small bug-fix exception in CONTRIBUTING.md. The changes cover watcher recovery, a GitHub failure classification needed to preserve permanent-error handling, and their regression tests. The new server-side `forbidden` reason does not change RPC contracts or add settings.

## Verification

`vp run check:changed` passed, including 154 tests across five files. Regression coverage includes a 30-minute outage followed by one CI-success notification, mixed activity/detail failures, resetting a prior failure count, and repeated forbidden reads stopping the watch. Provider tests distinguish HTTP 403 and exclusively permanent GraphQL refusals from retryable HTTP 503 and mixed or untyped GraphQL errors. The mixed-failure and GraphQL classification cases failed before their fixes. Seven focused requested-reviewer fallback/refusal tests also pass.
