fix(web): ensure question are always rendered#1825

Commit f10153b promoted `Layout` to be the top-level wrapper and altered how content outside the "installation settings context" is handled, redirecting to its own paths instead of rendering within the App component. As a result, question rendering across the app no longer works as expected. This change moves <Questions> component from <App> to <Layout>, which should restore the previous behavior.
As an step towards drop the PlainLayout from App.

As an attempt to avoid breaking it because changes at Layout component like it happens after #1825 As a side-effect, the Header component has been improved a bit by rendering the InstallerOptions dialog only when it can be open.
