Settings saves used a cached copy of settings.json, so an unrelated client save could revert an external edit before the file watcher reloaded it. A save could also replace malformed JSON with defaults.

Reload disk under the existing write semaphore before applying either an ordinary settings patch or a provider-instance mutation. Keep the decoded settings and parse failure in the same cache entry so reads retain their existing fallback behavior while saves reject unreadable JSON with the original cause. Derive migration trust from that parse result.

Built on @f4llenz's work in #18582, preserving the contributor commits and merging main. Fixes #18571. An external write during the short read-to-write interval can still be lost; this does not add cross-process locking or compare-and-swap behavior.

Verification

• 62 → 62 tests in serverSettings.test.ts: retained the two focused regressions and strengthened them to cover provider-instance saves, populated-cache failure, path/cause, and exact malformed bytes. Both regressions fail against pre-fix production; all 62 pass the rebuilt implementation.
• Full server-only typecheck, targeted formatting and whitespace checks pass. Targeted lint passes with two pre-existing test warnings.
• Live isolated Linux server, real WebSocket RPC: an atomic external edit survived a ten-save burst of unrelated patches. Both the API and disk retained the external value, and the unrelated patch persisted.
• A real settings update over a truncated 35-byte JSON file returned ServerSettingsError with operation read-file and the SchemaError cause. Disk remained byte-identical (SHA256 9649e75cf7400637750455bd0e26a492b1f83a921c6fb14b0b1007b08ac9d2ad).
• Independent Opus and Sol reviews: round 1 returned no findings from either reviewer.

This is a backend change; the proof is service tests and real RPC/disk results. The Settings error toast and native macOS, Windows, iOS, Android, and Electron desktop were not exercised. All clients use this existing server mutation path; no transport schema or client behavior changed.
