Problem

An agent that retries an MCP thread creation with the same request (for example after a dropped connection) is refused with provider_unavailable once the project turns that provider off, even though the original creation was already accepted and dispatching it again would only replay the recorded result. Driver-only delegation already lets an accepted retry through in this situation; thread creation did not.

Change

  • Thread creation checks for an existing receipt before resolving its target, and an accepted retry skips the project enablement filter, matching what delegation already does.
  • Because a retry can then resolve a different instance than the one originally created, the retry reads the recorded model selection back from the thread and uses it for the response and the follow-up message, so it reports what was actually created.

Scope and approval

No prior issue. This is a focused fix for an idempotency bug in the per-project provider switch from #17879: a retry of accepted work is refused, or would report a different provider than the thread really uses. It stays inside the thread creation path of the orchestrator MCP service and mirrors the retry handling delegation already has.

Verification

  • vp test run apps/server/src/mcp/OrchestratorMcpService.test.ts: 28 passed.
  • "retrying an accepted thread creation replays even after the project disables every candidate": with the fix reverted it fails with No instance of driver codex is enabled for this project.
  • "a retried driver-only thread creation reports the instance that was actually created" runs against the real orchestrator and thread management service, with the project disabling the parent's instance so creation picks an alternate. With the recorded-selection read reverted it fails with expected 'codex' to equal 'codex-alt'.
  • vp lint on the touched files and the apps/server typecheck are clean.
  • Not checked: a manual run through a real agent client.

Approvability

Verdict: Approved at b91c47a

Macroscope's review found this PR approvable — This is a narrowly scoped idempotency fix: accepted thread-creation retries now replay despite changed provider settings and report the originally persisted provider selection. Production behavior is isolated to the retry path, with both mocked regression coverage and a live test of the real orchestration and receipt services.

createThreads checks for an accepted request before resolving its provider target. For accepted retries, it skips the project-enablement filter and uses the created thread’s recorded model selection. Tests cover retries after provider candidates are disabled.

createThreads uses one command ID for receipt lookup, enablement validation, and dispatch. Accepted retries skip the project-enablement filter and use the created thread’s recorded model selection. Tests verify retry behavior and the returned and persisted provider selection.

Merge Risk: ⚪ Minimal · up to b91c4

Accepted thread-creation retries can replay after project provider settings change. No actionable merge-blocking risk remains after normal checks.

Security Architecture Review

Security architecture risk: ⚪ Minimal · up to b91c4

Previously recorded thread creation can replay after a project disables its provider. New follow-up work still requires a separate permission check, and retries use the thread’s recorded provider selection. No material security regression was identified in the changed behavior.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed exemption is scoped to receipt-bearing creation requests from an authenticated thread caller with orchestration capability and an active parent run owned by the caller’s provider instance. The child project comes from that parent rather than an independently supplied project target.

Security Findings and Attack Paths

  • inferred — The examined disablement-bypass path does not authorize fresh provider work merely because thread creation was previously recorded. Accepted creation replay returns stored events without planning new work; an unrecorded follow-up prompt remains independently gated against the recorded provider instance.

Trust Boundaries and Controls

  • observed — The caller controls the request key, but not the credential’s replay namespace. Issuance generates a random provider-session UUID and stores it with the environment, thread, provider instance, and capabilities. Bearer-token resolution supplies that stored scope; stable IDs encode its namespace alongside the request key and operation. Expiry and revocation remove access to the scope.

Resilience and Maintainability Implications

  • inferred — Interruption between creation and prompt dispatch can leave a durable child without a started prompt; the overall operation is not atomic. Retrying reuses the creation receipt, while the separate prompt command either replays its own receipt or checks current enablement before new work. This preserves failure containment rather than treating creation acceptance as blanket authority for the remaining steps.
