The pull request list shows a review badge and a Review filter for every host, but only the GitHub adapter fills reviewDecision. GitLab rows never carry one. An approved merge request shows no badge, and Review → Approved finds nothing even when GitLab reports the merge request as approved.

Change

The GitLab adapter now reads approvals for listed and summarized merge requests (getReviewDecisions), one GraphQL request per 100 merge requests, and sets reviewDecision on the same two reads the GitHub adapter does. If the approvals read fails for any reason, rate limits included, the decision stays unknown as before and the list still loads. The rows were already read by then, and the next read still reports a real rate limit.

The query only uses fields both GitLab editions have:

• approvalsLeft is Enterprise-only. Community Edition 19.2.9 rejects the whole query: "Field 'approvalsLeft' doesn't exist on type 'MergeRequest'".
• On Enterprise Edition, approved follows the project's approval rules. On gitlab.com, merge requests approved only by GitLab Duo report approved: false, so they read as review-required, not approved. The GitHub adapter currently counts any approval as approved; https://git.cafe/pingdotgg/t3code/pulls/25725 moves GitHub toward this reading.
• On Community Edition there are no approval rules, and approved was true exactly when someone had approved (checked on 40 merge requests). It never reads as review-required there. metadata { enterprise } tells the two editions apart.
• mergeabilityChecks reports the approval rule directly on both editions, but it added about 1.8 s per 50 merge requests on gitlab.com. approved plus approvedBy(first: 1) added about 0.3 s.

Scope

Submitted as a focused fix. The badge and the Review filter already exist for every host, and GitLab rows never match them. The change stays inside packages/source-control-gitlab: no contract, client or UI code changes.

Verification

• gitLabMergeRequestJson.test.ts: a decision table covering Enterprise and Community Edition, bot approvals short of the rules, and rules without approvals. GitLabPullRequestProvider.test.ts: list rows and the summary carry the decision, and a failed or rate-limited approvals read leaves it unknown. All six new tests failed on main before the fix; the package's 127 tests pass.

reviewDecisions goes through recoverRead, which still rethrows rate-limited, unauthenticated and missing-tool. In listChangeRequests it now runs in a flatMap after listMergeRequests has already succeeded, and in getChangeRequestSummary it sits in an Effect.all next to getMergeRequestDetail. So if only the extra GraphQL request gets rate-limited, the whole list or summary fails, where before this PR the same read returned the rows, just without a badge.

The body says rate-limit and auth errors reaching the caller is on purpose. For auth and a missing glab that seems fine, since the list read itself would hit the same error. Rate limits are different: this adds one request per listed page and one per summary on top of reads that already compete for GitLab.com's limit (#24190), and losing the list over a badge seems like the wrong trade.

Would it make sense to fall back to the empty map for rate-limited here too, for example with a local catch instead of recoverRead? If so, a rate-limited case next to "lists merge requests with an unknown decision when GitLab will not say" would pin it down. That test currently fails with a plain GitLabMergeRequestReadError (cause: null), which recoverRead already swallows, so it doesn't exercise the rethrow path.
