fix(web): align provider settings with shared settings rows

provider setup stacked its labels, status, and buttons while the rest of settings used left-hand descriptions and right-hand controls. antigravity setup now uses shared settings rows with short descriptions and stable space for runtime status, progress, and actions; provider variable and custom-model add actions sit alongside their labels/counts, and variable fields wrap on narrow screens.

verified in the real web app at 1440px, 768px, and 390px, in dark and light themes, including enable, install/cancel, sign-out confirmation, variable add/remove, and custom-model add/remove. 25 focused tests and the web typecheck passed; targeted lint passed with one existing effect-dependency warning. completing google authentication and the electron shell were not exercised; handlers and wire formats are unchanged. the final setup pass passed 11 existing setup tests, web typecheck, and targeted lint. a full download, extraction, verification, and installation kept all three setup rows at identical positions across 53 observed updates; the 390px download and 1024px stacked layout were checked too.

takeover verification at e5ae64f498: merged current main and resolved the model-header conflict, preserving bulk enable/disable and the custom-model action. all github checks pass; 29 focused tests, web typecheck, and scoped lint passed (one existing dependency warning). two independent gpt-6-astra reviews found no issues. the live client exercised all six real codex models through bulk hide/show and custom-model add/remove; the server persisted the change and cleanup. inspected the header at 1280px and 390px. existing review threads remain resolved.
