fix(claude): report provider-observed models after fallback - #27738
When Claude replaces an organization-restricted Opus request with Sonnet, T3 continues reporting the requested Opus model to parent agents. In the reported incident on 0.1.6-nightly.20261030.3904, SDK initialization and root response records identify Sonnet, while the completed child thread/run metadata identifies Opus. Successful child completion also omits the fallback notice from the delivered result.
The expected behavior is to distinguish requested configuration from provider-observed execution. No organization-policy bypass was observed.
Change
Capture the root model from Claude SDK system/init, assistant messages, and streaming message_start records. Ignore native-child frames, missing/synthetic values, and auxiliary modelUsage entries.
Persist observedModel on provider turns and project it onto the matching run as a field-only update, without writing a stale full-run snapshot. Preserve observations within the same attempt, including in shared client projections; clear them on restart and ignore reports from older attempts. Requested model selections and options remain unchanged.
Report the observed model through MCP task status and thread/run reads, with separate requestedModel and observedModel fields. Omit unknown observations from MCP JSON. Include both identities and current root provider notices in normal child completion text when models differ, and reconcile the completed parent task's model.
An unknown observation remains absent. Stored effort options describe the request; this change does not infer effective effort.
Scope and approval
This addresses one problem: incorrect execution-model attribution after a confirmed provider fallback. The adapter, contracts, persistence, shared projection, MCP, and completion changes carry the same observation through the existing reporting paths.
Issue #27705 is not yet maintainer-triaged. Maintainer confirmation of the failure and intended behavior remains outstanding under CONTRIBUTING.md's substantial-bug-fix requirement; no prior approval is claimed.
Related open work: #26309 handles restricted-model catalog prevention; #24126 and #23931 synchronize selection after session fallbacks; #24695 handles Bedrock model routing. This change retains requested configuration and reports provider-observed execution separately.
Verification
The initial red/green reproduction used base 848e352b48f982e0f1fe134aa9712d75ba7da3b9. Conflicts are resolved against upstream main 6fc9144a60c4a5f45089316ddd5f7371bafd3bf2; the updated head is 0a4c3a7e33fe6ffb5a5d94d640b7e4ed71734737.
The deterministic regression reconstructs the captured Opus request -> Sonnet init -> organization warning -> successful completion sequence. It exercises the production Claude adapter, orchestrator, disposable SQLite projections, and real MCP task-status service. Completion-format assertions live in the existing subagent projection tests. The MCP service reads the persisted replay projection through a test dependency; the fallback is not replayed over HTTP.
Before the fix, the regressions fail with missing Sonnet provenance and incorrect MCP attribution:
expected undefined to equal 'claude-sonnet-5-5'
expected 'claude-opus-5-5' to equal 'claude-sonnet-5-5'
The stored thread/run selection remains Opus. When the provider emits the captured organization-policy warning, normal completion includes both identities and the original explanation before the assistant result:
Requested model: claude-opus-5-5. Provider-reported model: claude-sonnet-5-5.
Provider notice: Model "claude-opus-5-5" is restricted by your organization's settings. Using claude-sonnet-5-5 instead.
All steps passed.
Notices are limited to the same run and current root node, excluding parented native-child notices. A differing alias or API suffix without a provider notice does not invent a fallback cause.
Focused reproduction commands:
vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts -t 'reports root model'
vp test run apps/server/src/mcp/OrchestratorMcpService.test.ts -t 'reports the child selection'
vp test run apps/server/src/mcp/OrchestratorMcpToolkit.integration.test.ts -t 'reports.*child results'
vp test run apps/server/src/orchestration-v2/SubagentProjection.test.ts -t 'orders assistant messages'
The post-merge verification passed 360 focused tests across the Claude adapter, provider event ingestion, MCP service, subagent completion projection, shared client projection, and automatic Claude completion-delivery integration suites. Coverage includes same-model runs, unfamiliar IDs, omitted/blank/synthetic telemetry, whitespace normalization, native children, and mixed Sonnet/Haiku usage. Init emits one observed-model session update.
Review regressions failed before the follow-up fix and pass afterward: lifecycle state is preserved, stale snapshots cannot overwrite observations, restart clears the observation, and delayed prior-attempt reports are ignored. A further focused run passed 334 tests across the adapter, projection, restart, and shared-client suites.
vp run check:changed 6fc9144a60c4a5f45089316ddd5f7371bafd3bf2 passed: 418 tests across 13 files, all 35 affected-package/dependent typechecks, formatting, lint, both knip passes, and GraphQL validation. Separate typechecks in the four directly affected packages and git diff --check also passed.
The completion projection regression failed before the reason follow-up because it omitted the captured organization-policy warning. All 12 owning-layer tests pass afterward, preserving assistant result IDs and excluding notices from other runs, previous roots, and native children.
The existing MCP toolkit integration now verifies the completed parent's stored task model and result, task-status delivery, follow-ups, and thread reads through the real MCP JSON validator. Its Codex fixture injects a normalized root-model observation before completion; this verifies orchestration/reporting, not Codex telemetry capture. It caught an invalid explicit undefined in MCP JSON before the omission fix. Alias and API-suffix cases preserve raw provider IDs without asserting that every differing ID is a fallback.
A disposable dev server started with --no-seed --home-dir <temporary-directory> and returned HTTP 200 from /.well-known/t3/environment, then was stopped. This is a startup/transport smoke check, not live fallback verification.
Not verified: live execution against an organization-restricted Claude account or browser/device labels. Existing incident threads, organization settings, and the live installation were left unchanged.

Walkthrough
The orchestration flow captures Claude’s provider-reported root model, preserves it across run and provider-turn projections, and exposes requested and observed models in thread, run, and delegated-task results. Restarted runs clear prior observations.
Run and provider-turn contracts add an optional observedModel. The Claude adapter extracts valid root model values, tracks them on live queries and turns, and includes them in provider-turn and session updates. Replay tests cover recognized, synthetic, empty, and absent model values.
Server and client projections preserve observed models when updates omit them and associate provider-turn observations with matching root nodes and active attempts. Restarted runs clear prior observations. Tests cover stale updates, attempt changes, and projection retention.
MCP thread, run, and delegated-task results include requested and observed model fields and use the observed model when available. Finalized tasks use the child run’s observed model when present, and subagent result text identifies differing requested and observed models and eligible system notices.

Clear the previous attempt’s model observation on restart.
When a steer restarts a run with a new model selection, restartedRun copies targetRun.observedModel into the new attempt. If that attempt reports no model, run and delegated-task results identify the previous attempt’s model as the current one. Projection storage also preserves an omitted observedModel, so omitting the field from the restart event will not clear it. Make observation retention attempt-aware, and leave the new attempt’s observation absent until its provider reports one. (raw.githubusercontent.com)

Fold this case into the existing restarted-child test.
The next test already sets different run and thread selections and calls taskStatus. Add the observed-model assertions to that case instead of maintaining a second fixture and service setup. (raw.githubusercontent.com)
As per coding guidelines, “Extend before you add. When an existing test or table covers the behavior, add a row or an assertion there.” The matching path instruction also requires this test rule.

Include the Claude fallback reason in subagentResultForRun.
Claude stores the fallback reason as a system_notice, but subagentResultForRun only selects assistant messages and assistant_message turn items. Parent task completion and MCP terminal summaries both consume this projected text, so they expose the model mismatch but omit the provider’s fallback reason. Include the applicable Claude system notice in the projected completion text.

When requested and observed IDs differ, completion now includes nonempty provider notices from the same run and current root node, alongside both model identities and the selected assistant result. This carries the original organization-policy explanation through parent task completion and MCP terminal summaries. Other runs, previous roots, and parented native-child notices are excluded. The code forwards the provider's words rather than inferring a fallback cause from differing aliases or API suffixes.
The existing completion regression failed before this change because the organization warning was absent. this does not meet the prior-approval requirement for a substantial bug fix. the pr explicitly states that #27705 is not maintainer-triaged. this change establishes requested-versus-observed model provenance across the adapter, contracts, persistence, shared projections, mcp reads and child completion, so the intended reporting behavior needs maintainer triage. the focused regression and integration evidence does not replace that decision.
have a maintainer confirm the failure and intended attribution scope in #27705, then link the decision here and align the implementation with it before requesting reconsideration. preserve the fallback report and existing verification evidence.
