fix(claude): follow the session model after a refusal fallback - #24126
When Claude safeguards flag a message, Claude Code retries it on the fallback model and swaps the session model (Opus 5.5 → Opus 4.8). T3 only shows the notice. The thread's model selection stays claude-opus-5-5, so:
the composer keeps showing Claude Opus 5.5 while the session runs on Opus 4.8,
the next resume opens the session on Opus 5.5 again,
picking Opus 5.5 again in the composer does nothing: the adapter still believes the live query is on 5.5, so it never calls setModel, and the session stays on 4.8.
ClaudeAdapterV2: on system/model_refusal_fallback with scope: "session" (or no scope, which the SDK says to treat as session), set the live query's selection to the fallback model and emit a new provider event, app_thread.model_selection.updated. scope: "local" (a subagent or side question fell back) changes nothing.
ProviderEventIngestor: write that selection to the thread under the thread lock. This reuses the lock/compare/write path the subagent model sync already had (pulled out as syncThreadModelSelection). A thread the user has since moved to another provider instance keeps its choice.
RunExecutionService: route the event to the run that owns the thread.
The model ID is resolved against the catalog the same way subagent models are, and the thread's other options carry over. direction is ignored: Claude Code 2.1.309 documents revert and sticky as "retained in the enum for SDK-consumer compat and no longer emitted".
No client or contract change beyond the provider event: web, desktop and mobile already render thread.modelSelection. Other adapters don't emit the event. Codex's model/rerouted for a main thread could use it later; I left that out to keep this to one problem.
Before and after
Both use real Claude Code 2.1.309 with the default switchModelsOnFlag: true, on a message Opus 5.5 flags as [cyber]. The turn is stopped right after the switch.
Before (main): the session moves to Opus 4.8, but the composer stays on Claude Opus 5.5.
03-before-composer-stays-on-opus-5-5.mp4
After: the composer moves to Claude Opus 4.8 when the switch happens.

Scope and approval
Fixes#24115. #24115 is marked needs more info because a fallback could not be triggered on demand. The recordings above are real fallbacks, and this is the frame Claude Code sent (prompt omitted, it relies on a personal skill):
After the AFTER run the stored thread has modelSelection.model = "claude-opus-4-8". The BEFORE threads still have claude-opus-5-5.
Verification
ClaudeAdapterV2.test.ts, new test "moves the thread to the model a session refusal fallback switched to": a local fallback emits nothing, a session fallback emits one app_thread.model_selection.updated with claude-opus-4-8, a next turn on 4.8 makes no live model change, and a turn back on 5.5 calls setModel("claude-opus-5-5"). On main it fails with expected [] to deeply equal [ { …(4) } ].
ProviderEventIngestor.test.ts, new test "moves a thread to the model its provider switched the session to": the event writes one thread.model-selection-updated, repeating it writes nothing, and a report for another provider instance leaves the thread alone. On main the event isn't handled and the test throws.
vp run check:changed: format, lint, knip, graphql, typecheck (20 packages) and tests (276 across the 3 changed test files) all pass.
Not checked: resuming after a server restart on a real fallback. The resume path reads thread.modelSelection, which the tests and the stored thread above cover.
Implemented with claude-opus-5-5 through Claude Code, running in T3 Code.
- Report Claude's session-scoped model_refusal_fallback as a new app_thread.model_selection.updated provider event
- Move the live query's selection to the fallback model, so picking the original model again calls setModel
- Write the thread's model selection under its lock, sharing the subagent model sync, and leave threads moved to another provider alone

Walkthrough
Claude refusal fallback events now update the thread’s model selection for non-local scopes. Provider event routing and ingestion synchronize the selection when the provider instance and previous selection match.

Summary
Report Claude fallback model changes packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The provider event type adds model-selection updates. Claude emits an update for non-local refusal fallbacks. Tests cover fallback scopes and subsequent model selections.
Route and synchronize thread model updates apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.test.ts
Run routing accepts updates for owned threads. Ingestion synchronizes the selection under the thread-command lock when the previous selection and provider instance match. Tests cover repeated reports and cases where the selection is not updated.
A narrow query-replacement race can select the wrong model, and the instance guard lacks effective regression coverage. Both are bounded fixes to make before or shortly after merge.

Explanation
The title "fix(claude): follow the session model after a refusal fallback" matches the main change. The PR makes ClaudeAdapterV2 handle session-scoped model_refusal_fallback frames by updating the thread's model selection. It then syncs that selection through ProviderEventIngestor and routes the event in RunExecutionService. The title is concise, uses a conventional commit format, and names the specific behavior.
Full details: Description check
Explanation
The description follows the repository template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section explains the expected behavior and the bug. The Change section lists the adapter, ingestor, and routing changes and explains why the client is unchanged. The Scope section links issue #24115 and explains the fallback frame. The Verification section names the focused tests with their results, the repo check run, and one item that was not checked. The template's optional agent-model line is present at the end. The description does not include a maintainer approval comment link, and the template asks for one when there is no prior approval. The objectives say approval came from a comment, but the description itself does not cite it. That gap is minor and does not make the description incomplete.
Full details: Linked Issues check
Explanation
[#24115] requires the thread to follow a Claude session fallback. The adapter handles session-scoped and scope-omitted system/model_refusal_fallback events, resolves fallback_model, updates the live query selection, and emits app_thread.model_selection.updated. It ignores local-scope fallbacks. The event is routed to the run that owns the thread. The ingestor writes the new selection under the thread lock only when the previous selection and provider instance still match. This preserves a newer user choice and supports composer display, resume behavior, and later setModel changes. Tests cover local versus session scope, repeated events, provider-instance changes, and subsequent model changes.
Full details: Out of Scope Changes check
Explanation
The changed files support [#24115]. ClaudeAdapterV2 handles the fallback event. ProviderEventIngestor synchronizes the thread selection and preserves existing subagent synchronization. RunExecutionService routes the event, and ProviderAdapter defines its event type. The added tests cover the new behavior. No unrelated adapter behavior, client change, or Codex model/rerouted handling is included.

merged current main without rebasing and preserved both conflicting adapter tests. fallback events now carry the full prior selection; ingestion compares it under the thread lock so a newer same-provider model or options choice is preserved. extended the existing regression case for both choices.
blacksmith verification passed: vp run check:changed origin/main (368 tests, format, lint, knip, graphql, and affected-package typecheck). two independent read-only reviews approved the scoped change. real claude/client fallback and restart/resume remain unverified on this vm; the contributor's recordings remain in the pr body.
Run the otherInstance case before the first update.
After first, the stale previousModelSelection check rejects otherInstance before the instanceId guard runs. Move the case before first so the test distinguishes the instance guard.

Use the liveQuery already bound for this frame.
The identity guard makes liveQuery non-null for the typed call paths. However, the fallback branch reads queryContext again. openQuery can replace that context while the frame is suspended. An old frame can then write its fallback selection into the new query.
♻️ Suggested fix
- const live = yield* Ref.get(queryContext);- if (live !== null) live.selection = compileClaudeModelSelection(modelSelection);+ liveQuery.selection = compileClaudeModelSelection(modelSelection);

verified exact head a43e78f738c423764a27e1effd9a7c80d221ac5a on linux, using repository playwright with /usr/bin/google-chrome, real dev servers, and real claude code/codex through the local oauth proxy. projects and threads were created through the app against a real git repository. no mocks or replay data.
reproduced on github/main (b50d8a1b15): claude emitted a native session fallback from claude-opus-5-5 to claude-opus-4-8, while the composer stayed on opus 5.5.
repeated the same prompt on this head: the native fallback immediately updated the composer to opus 4.8. opus 4.8 also refused the flagged request; this is the provider's refusal, not a successful answer claim.
a follow-up retained opus 4.8. after stopping and restarting the server with the same isolated data, the same native claude session initialized with model: claude-opus-4-8.
selecting opus 5.5 again in the composer emitted native query.set_model with claude-opus-5-5. the flagged context then caused another real fallback, and the composer followed it again.
codex gpt-6.1-sol completed real app turns on both main and this head. blacksmith passed all 313 tests in ClaudeAdapterV2.test.ts and ProviderEventIngestor.test.ts. no code changes were needed.
native desktop and ios/android clients were not available on this linux vm. local-scope subagent fallback, absent-scope older clis, concurrent model/options/provider changes, and query-replacement races were not induced against a real provider. existing tests cover local scope and newer selection preservation; those results do not substitute for live concurrency proof. the flagged conversation continued receiving provider refusals, so successful post-fallback answer generation was not demonstrated.
