fix(client-runtime): expire missing file metadata per thread - #30606
When an agent mentions a path before creating it, thread metadata can retain the missing result indefinitely, leaving a file icon on a newly created folder.
Expire retained missing results after 60 seconds and retry mounted missing results at that interval. Retries stop once metadata is found or the reader is disposed. Known file and folder metadata keeps its existing retention.
Validation: all 12 metadata tests and vp run check:changed pass. The regression test first failed on the mounted stale result, then passed without an explicit refresh after expiry. Browser verification was blocked by HTTP 431 errors.
Macroscope's review found this PR approvable — Previously missing files can now be rediscovered within the same thread through periodic metadata refreshes. The change is confined to read-only metadata caching, with existing positive-result retention and session boundaries preserved.
You can add or adjust custom eligibility rules. Learn more.
Retained thread metadata now reuses cached non-null entries after expiry, but reuses cached missing-path results only while they are unexpired. A test verifies that expired misses are retried, denied access returns an authorization error, and refreshed metadata is returned.

Summary
Expire cached misses and verify refresh packages/client-runtime/src/state/fileMetadata.ts, packages/client-runtime/src/state/fileMetadata.test.ts
Retained non-null metadata remains reusable after expiry. An expired cached miss triggers a new metadata query. The test checks denied access, refreshed directory metadata, retained known metadata, and requested paths.

The runtime fix resolves stale missing metadata and correctly cancels retries on disposal; only a small regression-test improvement remains.

Explanation
The title names the package (client-runtime), the fix type (fix), and the main behavior change (missing file metadata now expires per thread). It matches the changeset, which expires retained missing results after 60 seconds and retries mounted missing results at that interval. The title is specific and follows the conventional commit style that the template recommends.
Full details: Description check
Explanation
The description states the problem: a retained missing result can leave a file icon on a newly created folder. It states the change: missing results expire after 60 seconds, and mounted missing results retry at that interval until metadata is found or the reader is disposed. It includes verification details (12 metadata tests, check:changed, a regression test that failed first and then passed) and a model/harness note. It is missing the template's required 'Scope and approval' section. It has no link to a triaged issue or maintainer-approved discussion, and no explanation of why the fix qualifies without one. The Problem section also lacks an explicit expected behavior statement and reproduction steps. The Verification section notes that browser verification was blocked, which the template asks for, so that part is covered.
Resolution
Add a '## Scope and approval' section. Link the triaged bug issue or the maintainer-approved discussion, including the approval comment. If no issue exists, explain why this small, focused fix of an obvious bug qualifies without prior approval. Also rename or restructure the description to use the template headings '## Problem', '## Change', and '## Verification'. In '## Problem', state the expected behavior and reproduction steps, such as creating a folder after an agent mentions its path. Keep the existing verification details and the browser-verification limitation under '## Verification'.

Revalidate expired misses for mounted atoms.
The expiry check runs only when execute runs. A later reader of an already mounted atom can receive the cached null without rerunning it. (raw.githubusercontent.com)
A newly created directory can therefore keep its file icon after 60 seconds. (raw.githubusercontent.com) Add miss-specific revalidation and test a second lookup without an explicit registry.refresh.

Confirmed the mounted-atom finding in review 5763005544 with a failing regression test. Fixed in 00576c4: mounted missing results retry after 60 seconds using Atom.withRefresh, and stop scheduling retries when metadata is found or the reader is disposed. The test no longer explicitly refreshes after expiry and checks that a subsequent interval causes no additional filesystem request. Known-entry retention is unchanged. All 12 metadata tests and check:changed pass.

The current timer advance runs before Effect.scoped disposes the registry. Add an explicit h.registry.dispose() and advance another 60-second interval. Assert that h.batches does not change. This covers the disposal behavior changed by this fix without adding a production change.

Update the test around h.registry.dispose to explicitly dispose the registry

assert that h.batches remains unchanged from its pre-disposal snapshot. Keep
this change limited to the test; no production changes are needed.

Reviewed the disposal-test nitpick in review 5763133180. Leaving the proposed assertion out: at that point the missing path has already resolved to a directory, so no retry timer remains to cancel. Disposing the registry there and observing no requests would not establish cancellation behavior. Timer cleanup uses Atom.withRefresh and the zero idle TTL rather than a custom timer implementation. The focused regression verifies mounted miss refresh, recovery after an error, retention of known metadata, and no further filesystem requests after resolution.
