Startup recovery and thread settlement reconstructed canonical message ordering by scanning turn-item history, although these decisions only need message identity and notification metadata. Long threads made these reads expensive.

Add a primary-key message lookup that returns complete records by ID. Recovery uses it for pending runs, including upstream's recovery of unstarted runs. Settlement loads only queued-run messages. Canonical ordered projections and the streaming-message recovery selector keep their existing behavior; no schema or index change is needed.

Validation covers complete records, duplicate and absent IDs, thread isolation, reader routing and uncommitted writer visibility. Existing recovery and settlement decision tests also pass. A task-owned 100,000-item fixture confirms the new lookup avoids history scans. Native query timings are diagnostic evidence, not an estimate of whole-startup savings.

Scope and approval: this is a focused performance bug fix for measured startup recovery and settlement scans. Those callers consume message identity, not ordering, so reconstructing all historical ordinals is unnecessary. The fix adds no feature, schema change, or unrelated optimization.

Implemented with GPT 6.1 Sol through the Codex harness.

Walkthrough

The projection store adds a read method for fetching messages by thread and message IDs. Settlement and recovery use it to retrieve messages referenced by queued runs. Settlement also checks that an automatic queued message has the user role.

Changes: Queued-message reads

Layer / File(s) Summary
Projection store message lookup
apps/server/src/orchestration-v2/ProjectionStore.ts, apps/server/src/orchestration-v2/ProjectionStore.test.ts
The store adds getMessagesById, which deduplicates requested IDs, scopes reads to a thread, and returns decoded messages keyed by ID. Tests cover reader routing, uncommitted writer visibility, and bounded selected reads.
Queued-run message callers
apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/ProviderRuntimeRecoveryService.ts, apps/server/src/orchestration-v2/ProviderRuntimeRecoveryService.test.ts, apps/server/src/orchestration-v2/ProviderTurnControlService.test.ts
Settlement and recovery use getMessagesById for messages referenced by queued runs. Settlement requires the fetched message to have the user role before detecting automatic messages. Test doubles implement or reject the new lookup.

Approvability

Verdict: Approved at b298645

Macroscope's review found this PR approvable — Recovery and settlement now fetch only relevant messages by ID instead of loading ordered message projections. The change preserves existing queue ordering and settlement safeguards while keeping database access thread-scoped and read-only.

Make the queued-restart getMessagesById mock check the requested thread and IDs.

The mock returns the fixture message for any call. A caller that requests the wrong thread, a wrong message ID, or no IDs still gets that message, and the recovery assertions still pass. Filter the fixture by the requested thread and IDs, and record the lookup so the test can assert what the caller requested.

Suggested fix
-          getMessagesById: () =>
-            Effect.succeed(new Map(projection.messages.map((message) => [message.id, message]))),
+          getMessagesById: (threadId, messageIds) => {
+            requestedMessageLookups.push({ threadId, messageIds: [...messageIds] });
+            return Effect.succeed(
+              new Map(
+                projection.messages
+                  .filter(
+                    (message) =>
+                      threadId === projection.thread.id && messageIds.includes(message.id),
+                  )
+                  .map((message) => [message.id, message]),
+              ),
+            );
+          },

Declare requestedMessageLookups next to committedInput. After reconcile("startup"), assert that the recorded lookup used projection.thread.id and [MessageId.make("message_queued_restart")] in scenarios where the run is pending.

Note

Generated by GPT 6.1 Sol

The recovery test double now filters by the requested thread and message IDs. The existing recovery outcome assertions will therefore catch incorrect lookup arguments. I omitted separate call-recording assertions to keep this test focused on recovery behavior, in line with the repository's rule against mock-call mirrors. The projection-store tests already verify lookup semantics and writer visibility.

The PR description now explains why this is a focused performance bug fix. No implementation scope was added.
