chore(deps): Effect 4.0.4 and OpenCode 2.0.28, dropping the patches they made unnecessary

Problem

We pin Effect 4.0.2 and carry three patches to it, plus three to OpenCode 2.0.23 that port it to stable Effect. Effect 4.0.3 and 4.0.4 upstreamed two of ours, OpenCode 2.0.28 targets stable Effect itself, and 4.0.4 adds effect/graphql, which we want for GitHub.

Change

• Bumps effect and every @effect/* package to 4.0.4.

• Drops patches/effect@4.0.2.patch: Effect now has its own onPingTimeout option (Effect-TS/effect#8976). packages/client-runtime/src/rpc/session.ts now passes the hook to RpcClient.makeProtocolSocket({ onPingTimeout }) instead of setting it on ConnectionHooks. The session still tells "stopped responding" apart from "disconnected" the same way.

• Drops patches/@effect__platform-node-shared@4.0.2.patch: 4.0.4's NodeChildProcessSpawner no longer runs taskkill for a leader that has already exited.

• Moves the @effect/vitest patch to 4.0.4. It still swaps vitest for vite-plus/test.

• Crypto.randomUUIDv4/v7 became functions in 4.0.3 (Effect-TS/effect#8935), so every call site now calls them, and one test fake was updated to match.

• Bumps @opencode/{client,protocol,schema} to 2.0.28 and drops their three patches. 2.0.28 targets stable Effect 4.0.1: it no longer imports effect/unstable/*, Encoding or Schema.isStartsWith, and its statics helper no longer replaces Schema.make. The overrides that point OpenCode's pinned effect at our single copy stay, and the lockfile has no second Effect.

Verification

• tsc-rs --noEmit passes in all 29 packages.
• Test suites: source-control-github (352), source-control-core (46), provider-core (193), effect-acp (74), shared (1,571), contracts (661) and client-runtime (2,093) all pass.
• OpenCode: provider-opencode tests pass (198), and so do the server's OpenCode adapter (99) and orchestrator replay (13) tests. OpenCode2Client.live.test.ts passes against a real opencode v2.0.28 server.
• apps/server: 5,959 tests pass and 9 fail. All 9 also fail on clean main on this Mac, from macOS /var vs /private/var paths (invocation, ServerEnvironment, Antigravity fs, externalLauncher, AgentSessionScanner, UsageService). I confirmed storageCleanup the same way, by running it on origin/main in a separate worktree.
