fix(server): changing Claude effort no longer fails while background agents run

Changing reasoning effort on a Claude thread (for example High to Ultracode) while background agents or workflows ran failed every turn with "Claude is still running background agents or commands, and this model or setting change would end them." The model was the same. Only the effort changed.

Effort was part of the Claude adapter's selection key, so any effort change closed the Claude process and opened a new one. Background agents run inside that process, so the guard from #14726 refused the turn.

Now a change to model, effort, Ultracode, or Fast mode is applied to the running process with the SDK's setModel and applyFlagSettings. The process stays alive and background work keeps running. A thinking on/off change still needs a new process, because thinking is a launch flag. So does clearing an explicit effort (switching to a model without effort, such as Haiku 4.5), because a live reset goes to the model default instead of the user's configured effort. Both are still refused while background work runs. If a live change fails partway, the next turn sends the model and flags again.

Proof

• New adapter test: effort plus Fast, then a model change, go to the running process. A thinking change and a switch to Haiku 4.5 are still refused. On main this test fails with the same error users see.
• Real Claude Code 2.1.296 (control requests only): high to xhigh with Ultracode on, back to high with Ultracode off, then Sonnet at low. get_settings reported each value, all on one process.
• ClaudeAdapterV2, ClaudeAutomaticDelivery, and ProviderTurnStartService tests pass (272). Server typecheck is clean.

Known follow-ups (not in this PR)

• A refused turn is replayed as a "missed" run on the next turn. When the context is more than 75% full, the replay hits the handoff budget and fails with "Insufficient context allowance for the provider handoff."
