Project hooks now reload and request trust when a resumed thread switches dcode into another workspace.
Cwd switches previously reused the launch workspace's hook runtime and trust decision. The new TUI gate fails closed, keeps explicit-only policies non-interactive, binds allow-once grants to the approved file contents, and restores the prior runtime on rollback.
