Fresh Dissolution closeout — post-DECISIONS-nuke tag authority - #3409

After #3389 removed DECISIONS.md and dissolution-inventory.md, refresh every v4 .dag coproduct/gated mark to slug-only authority (merge-base map where available, live-tail fallback otherwise). Adds refresh_v4_dissolution_tag_authority.py for deterministic regen; strict_deprose --check unchanged. Co-authored-by: Cursor <cursoragent@cursor.com>

Fix the refresh script to preserve CP-1b parenthetical anchors when stripping ledger prefixes.
