fix(shared): thread opens stop waiting behind every other SQLite read - #110
Conversation
A read-only query waits for an unrelated open read-only transaction because every reader query shares one reader permit. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
All reader work shared one worker behind one permit, so a read-only transaction held every unrelated read behind it. Reads now lease one of up to four reader workers, started lowest slot first and only under contention. A transaction keeps its leased worker until its scope closes. Effect's Semaphore lets a fresh acquire take a permit ahead of a woken waiter, so under steady load an old read could wait behind dozens of later ones. Reader slots now go to waiters first come, first served: a release hands its slot straight to the oldest waiter, and a caller interrupted after taking a slot passes it on. [minor] Add cancellation coverage for the new reader queue · test-gap
The new queue's cancellation cleanup has no focused tests. Existing cancellation tests use an in-memory database and bypass reader workers, so they cannot detect lost reader slots. This misses the repository rule: "Backend behavior changes ship with focused tests for that behavior."
When it breaks: A future regression in queued-waiter removal or interrupted slot handoff could lose pool capacity without failing the current tests.
Evidence (level 2: pointed at the code): nodeSqliteClient.test.ts:242-312 adds concurrency and FIFO tests without interrupting a reader. Cancellation tests at :414-495 configure filename: ':memory:', bypassing makeReader through nodeSqliteClient.ts:1156-1157. The untested custom cleanup is at nodeSqliteClient.ts:530-540. Effect's callback cancellation invokes the supplied cleanup, rather than managing this slot itself, in packages/shared/node_modules/effect/src/internal/effect.ts:1172-1205. The base version has no custom slot queue. This confirms a test gap, not a demonstrated leak in the current implementation.
Suggested fix: Add deterministic file-backed reader-pool tests that interrupt a queued read and a read during slot handoff, then verify that subsequent reads can occupy all four slots. Coordinate through Deferreds and a controlled scheduler.

Verify this finding against the current code before changing anything. In packages/shared/src/nodeSqliteClient.test.ts:267-312, extend reader-pool coverage to cancellation. Existing interruption tests at lines 414-495 use in-memory databases and never exercise the custom queue cleanup in nodeSqliteClient.ts:530-540. Add deterministic file-backed tests for queued cancellation and interrupted slot handoff, and verify all four reader slots remain usable afterward. Use Deferreds and controlled scheduling, without sleeps or polling.
Tier 7/10 from claude-opus-5-5: Concurrency change on the SQLite read path that every thread open and message uses; a slot leak or ordering bug would show up as stalls or deadlocks that aren't noticed right away
Complexity high: Adds a custom FIFO reader-slot pool with interruption and handoff semantics, worker lifecycle for each slot, and a reworked borrower and transaction acquirer, all running under uninterruptible masks and scheduler-yield constraints
… readers turn off Also covers a queued read interrupted before it gets a reader. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
A release ran the next waiting read inline, and with the readers off that read released straight away, waking the next one inside it. The queue drained as one recursion: the stack grew with its length and the reads reached the writer in reverse. Once the readers are off, release now wakes every queued read from one loop. Tier 7/10 from claude-opus-5-5: Concurrency change on the SQLite read path that every thread open and message uses; a mistake here (leaked slot, deadlock, misrouted read) would stall or break reads in ways that aren't obvious right away
Complexity high: Custom FIFO slot admission with interruption handoff, uninterruptible masks, and a drain to the writer when readers turn off, layered over a worker pool. Async ordering is subtle here.
Opening threads and sending messages on stl-wsl kept stalling on "Syncing messages..." for seconds to minutes while agents ran. The server reads its 43 GB state database through one SQLite reader connection behind a one-permit lock, and every read waits in that one line. Over about 6 minutes of traces on Oct 26, 1,284 read transactions waited a median of 0.7 s, a 90th percentile of 9.3 s and at worst 28 s. One 0.02 ms lookup for a thread open waited 53.4 s.
Two things made it slow:
Semaphorewakes waiters through a scheduled task, and any fiber that asks before the woken waiter runs takes the permit. During that 53.4 s wait, 70 reads got the reader, and 41 of them had asked after it.This patch carries upstream pingdotgg#24762, a lazily started pool of four reader workers, merged onto upstream
maind73fd12. On top of that it hands reader slots out in arrival order. A release passes its slot straight to the oldest waiting read, and a new read takes a free slot only when nobody is queued. A read cancelled while queued leaves the queue, and one cancelled after getting a slot passes the slot on. If the readers turn off (a worker can't open the database) while reads are queued, all of them move to the writer in arrival order. The writer's lock is unchanged.Merge notes. The conflict was the reader acquire in
nodeSqliteClient.ts; pingdotgg#24762's version won, and main's threeeffectSucceedWithVoid:offcomments moved onto the matching lines. Main's newereffectFnOpportunitylint (pingdotgg#25099) rejected pingdotgg#24762'sborrowWriterarrow, so it's nowEffect.fnUntracedwith the same behavior. Apart from that, the file differs frommainby exactly pingdotgg#24762's diff.Evidence: the ordering test.
serves a waiting read before a read requested after itfills all four readers, queues read "earlier", then releases one reader and asks for read "later" in the same turn. On the pingdotgg#24762 merge it fails three runs out of three:With the fix it passes. Review round 1 found that, once the readers turned off, queued reads drained as one nested recursion and reached the writer in reverse.

A harness drives the real client with its reader workers on a temporary WAL database. Read transactions arrive at a steady rate and hold their connection across a 10 ms async gap, every fifth arrival is a standalone lookup, and the event loop is kept busy 3 ms of every 6. Each row is one 10 s run, about 840 reads:
main, one readerThe pool is what removes the waits. The harness shows no measurable difference from in-order admission at these loads; the case for it is the failing test above and the 41 later reads served first in the production trace. This hasn't run on the live server yet; that happens after the fork build.
Review tier: 7/10; concurrency on the read path every thread open and message uses
Two-way door: removing the patch line drops it from the next fork build
Blast radius: every SQLite read the server makes
