docs(crew): reconcile channel-first direction (D-037), VISION banner, open-decision hygiene

Decision number: this PR takes D-037, not the issue body's "D-028". D-028/029/030 are PR #120, D-031/032 PR #124, D-033 PR #127, D-034 PR #129, D-035 PR #134, D-036 PR #128, D-039 is #135. The issue's "D-028" label is superseded by D-037; every reference in the diff uses D-037.

Rebased onto latest origin/main, which now includes #123 and #138. The only conflicts were in docs/crew/DECISIONS.md between PR #124's D-031/D-032 and main's D-039; resolved by keeping all three in sequence (D-031 → D-032 → D-037 → D-039).

Definition of Done → change that satisfies it:

Docs-only. Verification the issue asks for:

• grep -rn "control surface" docs/ *.md → only VISION.md still contains the phrase, immediately followed by its **Superseded by D-037** marker.
• pnpm --filter buzz check and pnpm --filter buzz typecheck pass; git diff --check clean.

CI: NuncioCrew Gate green. Skipped lanes (Desktop Smoke E2E, Project Relay, Desktop Rust, macOS ARM Package, buzz-acp, Desktop Fast) are the known-broken/advisory lanes from issues #109 / #110 / #135; not affected by this docs-only change.
