Address Codex review feedback from PR #8398:
Re-add caller transcript persistence in the relay-server fallback path when the controller is not yet initialized, so early utterances are not lost from conversation history.
Update voice_call_control prompt rules to reference the replacement text instead of the raw [CALL_OPENING] marker, which gets replaced before reaching the model.
