Address Codex P2 + Devin on #26265. (1) agent-wake's new timeout branch returned {invoked: false} — the same sentinel as conversation-not-found. CLI surfaced false errors when a busy conversation stayed >30s. Now disambiguate with a reason field and update consumers. (2) proactive-chat-e2e timing was relaxed 20x (100ms→2000ms) which masks regressions; tighten to 500ms — still CI-safe but catches genuine perf drift.
