A request at or below cache_threshold went straight to the expected-wait score over every healthy worker: a miss was "not a spill", so it skipped the count-pressure gate every other path applies. The score keeps no count bound of its own, and its rate term divides by gen_throughput, which an engine never clears: an idle SGLang worker reports the ~0.35 tok/s left by its last one-token health generation, so one dispatch read as thousands of seconds and a burst of unrelated prompts piled onto the other worker until the next poll (2:30 on mock workers, 29:3 on real ones, with free slots on the loser the whole time). Route a miss, like a spill, among the healthy workers under the gate, on the tree, event-driven and hash paths; the KV-pressure fallback stays ungated, since a count gate there could steer onto a KV-saturated engine. Score against the rate summed over ranks with requests running, so an idle rank's stale gauge counts for nothing and the worker scores at the default throughput, like one that reports no rate. Closes #2804

…t the fleet rate Hash mode checked KV pressure only after its untokenized and head-only early returns, so those two misses (every HTTP-router request, in hash mode) kept the ungated expected-wait selection. Check KV pressure first, as the tree path does, then route both through the count-pressure gate like any other miss. A worker with no live rate was scored at the flat default throughput, which beside busy peers made it look slower per dispatched token than they are: at 8000 tok/s live against a 2000 default, the busy worker took four of every five dispatches. Score it at the fleet's nominal rate, the mean live rate among the candidates, which is the default only when no candidate has one. Refs: #2804
Since the scorer falls back to the fleet's nominal rate, the configured default only applies when no candidate reports a live rate; the CLI, config and Python help strings still said "when a backend reports none". Refs: #2804

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick winQualify the miss-burst bound for KV pressure.
When KV pressure triggers, non-hash routing sends requests to
select_worker_fallbackbefore the miss gate. Hash routing also checks KV pressure first. Both fallbacks select from all healthy workers, so no-holder requests do not receive the documented count-margin bound. Qualify both the policy comment and theCacheAwareConfigdocumentation. Keep the existing KV-priority behavior.Suggested fix

The count-pressure gate bounds a burst of misses on the tree, event and hash paths, but the fleet-wide KV-pressure fallback that runs ahead of them selects from every healthy worker; say so where the bound is described. Refs: #2804

Before my rule-17 review: this head no longer merges with main. Since fad0f55 (the kv_index chain index), a merge with main 9a28cb1..fad0f55 conflicts in From reading the diff at 8c70ec2, the approach looks right:
One note, not this PR's: a multi-rank worker with one idle rank and one busy rank is scored at the busy rank's rate alone. So it reads as slower than a peer whose ranks are all busy, as it did before this change. Counting an idle rank at the busy ranks' mean would fix that, separately. At the rebased head I'll run fmt, |
Resolves the conflicts with fad0f55 (the kv_index chain index) in cache_aware.rs and least_load.rs, keeping main's structure: - select_final_from_affinity routes a miss and an all-gated spill among the ungated candidates; resolve_selection reaches it for the event-driven miss, so the explicit gated fallback there is gone. - drain_rate takes the candidates' nominal rate and prices a report with no live rate (none, or only idle ranks) at it; score_inputs folds live_throughput for that nominal rate.

Merged main at fad0f55 into this branch in fc5995b and resolved both files. It is a merge commit rather than a rebase: this session cannot force-push, and the sign-off re-author that DCO still needs will linearize the history anyway. What the resolution kept from main and from this PR:
Gates on the merged tree: On the multi-rank note: agreed. A worker with one idle rank and one busy rank is scored at the busy rank's rate alone, as before this change, so it reads slower than a peer whose ranks are all busy. Counting an idle rank at the busy ranks' mean is a separate change; I can take it as a follow-up. Generated by Claude Code |

CI note on fc5995b: three jobs failed on tests whose outcome is a remote MCP server's or the model's on the day, none on code this diff touches. The other 45 jobs of the run are green. Update 21:56Z: all three passed on the single re-run (attempt 2: openai-responses, e2e-1gpu-responses, go-bindings-e2e), and the run is green. The record of the first attempt stays below.

Review of #2810 at fc5995b: approve. I checked it against the routing-policy checklist.
What it does:
- Misses go under the count-pressure gate. Hits already spilled at that gate.
- In tree and event mode, a request with no holder now routes among the workers under the gate (
ungated_candidates). Expected wait alone keeps no count bound, so a burst of misses piled onto one worker while another had free slots. - In hash mode, KV pressure is checked first, as the tree path does. The untokenized, head-only and no-holder misses then go through the same gate.
- In tree and event mode, a request with no holder now routes among the workers under the gate (
- Idle workers are scored at the fleet's nominal rate. A worker with no live rate is scored at the mean live rate of the candidates that have one, or at
default_throughputwhen none does. At a flat 2000 tok/s beside a peer at 8000, the busy worker took four of every five dispatches.
What I checked:
- Both paths: HTTP and gRPC reach the same
select_worker. The PR adds no state, so nothing new needsSend + Sync. - An empty selection:
ungated_candidatesfalls back to every healthy worker when the gate leaves none, so a selection never returns no worker.healthy_indicesalready holds only healthy workers whose circuit breaker can execute. - The tests: each mode has a burst test asserting 4/4 picks under rates 100× apart, at a gate set to the mean: tree, event, hash, and hash untokenized. A test puts an idle worker at its busy peer's rate (4/4). The tests' loads now mark a request running, so their rate gauges count as live.
- The docs: the operator help for
default_throughputmatches the new fallback, both for the CLI and the Python binding. - Main moved: #2866 added decision tracing to
cache_aware.rs. The merge with main a7e2828 is clean, and the gates below ran on it.
Gates. I ran them on fc5995b merged on main a7e2828, with Rust 1.98.0, its own target dir and -j 2; each exited 0:

A note, not a blocker: a multi-rank worker with some idle ranks is still scored at its busy ranks' summed rate, which undercounts what its idle ranks would drain. That was so before this PR, and the PR doesn't change it.
Description
Problem
With
--policy cache_aware, a request whose prefix match is at or belowcache_thresholdgoes straight to the expected-wait score over every healthy worker:select_final_from_affinitytreats a miss as "not a spill" and skips the count-pressure gate (candidate_requires_spill) that bounds every other path. The score keeps no count bound of its own. Its queue term is a poll old and its in-flight term istokens_dispatched_since_poll / gen_throughput, wheregen_throughputis the engine's last decode-interval rate, never cleared: an idle SGLang worker reports the ~0.35 tok/s left by its last one-token/healthgeneration, or0(replaced bydefault_throughput). One dispatch scored against the residual reads as ~2800 s, so a burst of unrelated prompts follows whichever worker reported the larger stale number until the next poll. Two 16-slot workers split a 32-request burst 2:30 (mock) and 29:3 (real SGLang 0.5.21), with the loser idle the whole time.Closes #2804
Solution
Two changes, one per cause:
select_final_from_affinitynow routes a miss, like a spill, among the healthy workers under the count-pressure gate (ungated_candidates), so a burst of misses is bounded by the samebalance_abs_threshold/balance_rel_thresholdmargins a hit spills at. The event-driven no-overlap fallback and every holder-less hash-mode branch (no holder, untokenized, head-only) get the same bound; hash mode now checks KV pressure first, as the tree path does, so those branches are gated only when the fleet is KV-balanced. The KV-pressure fallback itself is untouched: it exists to act on KV, and a count gate there could steer onto a KV-saturated engine.LeastLoadPolicyscores againstlive_throughput, the rate summed over ranks with requests running. A rank with nothing running contributes no rate, so a worker with none is scored at the fleet's nominal rate (the mean live rate among the candidates), exactly like one that never reports a rate; that isdefault_throughputonly when no candidate has a live rate. An idle worker beside busy peers therefore reads as fast as they are, not slower, and the idle residual no longer feeds the nominal rate used for workers without a fresh snapshot. Same forpower_of_two, which shares the scorer. Not done here: folding
num_running_reqs/max_running_requestsinto the score itself. The score models the wait to drain the waiting queue; running requests are the batch the throughput term already measures, so counting them again double-counts. The gate is what bounds the in-flight spread.Changes
model_gateway/src/policies/cache_aware.rs:ungated_candidates;select_final_from_affinityuses it for both the spill and the miss; the event-driven fallback and the hash-mode no-holder, untokenized and head-only branches pass the gated set, with the hash-mode KV check moved ahead of them;hash_expected_waittakes the candidate set. Tests: a burst of misses on the string-tree, event-driven and hash paths (tokenized and untokenized) splits 4:4 with the gate at the mean despite a 100× live-rate skew; the issue's two-idle-worker mock splits 4:4 without any gate.model_gateway/src/policies/least_load.rs:live_throughput; the score and the nominal throughput use it, and a report with no live rate is scored at the nominal rate. Tests: an idle rank's gauge counts for nothing, a busy rank's does; two idle workers reporting0.36and0tok/s water-fill 4:4; an idle worker beside a busy 8000 tok/s peer water-fills 4:4 instead of taking one dispatch in five. Fixtures here and inpower_of_two.rsmark their one rank as running so the throughput they state stays live.model_gateway/src/policies/mod.rs:balance_abs_thresholddoc mentions the miss bound.Test Plan
Before (both fixes disabled, same tests): with
balance_abs_threshold: 0, balance_rel_threshold: 1.0and w1 reporting a 100× higher live rate, 8 misses on the string-tree, event-driven and hash paths each split 7:1 (the first pick is a random tie, every later one goes to w1); two idle workers reporting0.36/0tok/s split 8 misses 1:7, on the cache-aware miss path and on bareleast_loadalike. After: 4:4 in every case. With
