🔴 Important: This closes the current message item unconditionally, even when it was already closed normally — which destroys the text it is supposed to preserve.
process_chunk never clears current_message_output_index / current_item_id after the finish_reason == "stop" | "length" branch runs; it only calls complete_output_item(output_index) and mem::takes accumulated_text. So after a normal stop, the emitter is left with:
current_message_output_index = Some(i),current_item_id = Some(id)has_emitted_output_item_added = has_emitted_content_part_added = trueaccumulated_text = ""(already taken)output_items[i].item_data = Some(<full message with the real text>), statusCompleted
Failure scenario (regular non-MCP path, process_and_transform_sse_stream): backend streams the full answer, sends finish_reason: "stop", then the connection drops (or the chat layer emits an error frame on the trailing usage chunk) before data: [DONE]. failure is set → emit_failed → this function runs again on the already-closed item and:
- re-emits a second
output_text.done/content_part.doneladder withtext: "", - re-emits
output_item.donefor the sameoutput_indexwith"status": "incomplete"and"text": "", and emit_output_item_done→store_output_item_dataoverwrites the stored completed item, so theresponse.failedobject shipsoutput[0].content[0].text == "".
The client that just received the whole answer via deltas gets a terminal object with the text erased, and it diverges from what finalize_failed persists (the accumulator still holds the full content_buffer) — breaking the "what the caller persists is exactly what the client was shown" invariant this PR documents.
Guarding on the tracked item still being in progress fixes it (and makes the early-return also clear the stale ids):
let (Some(output_index), Some(item_id)) = (
self.current_message_output_index,
self.current_item_id.clone(),
) else {
return;
};
// Already closed by the finish_reason ladder — do not re-close or the
// stored item is overwritten with the drained (empty) text.

🔴 Important: On the harmony path the partial output is not attached, so this comment ("terminal-fail with the partial output attached") and the PR's stated contract don't hold here.
emit_failed → close_open_message_as_incomplete reads the emitter's current_message_output_index / current_item_id / has_emitted_output_item_added. Harmony never sets those: HarmonyStreamingProcessor tracks the open message item in local variables inside harmony/streaming.rs (message_output_index, message_item_id, has_emitted_content_part_added — see harmony/streaming.rs:834-880) and only calls emitter.allocate_output_index / emit_output_item_added / emit_text_delta.
So when process_responses_iteration_stream returns Err mid-stream:
current_message_output_indexisNone→close_open_message_as_incompletereturns immediately, no done ladder is emitted, and the message item is left dangling with nooutput_item.done.- That item's
item_datawas never stored (onlyemit_output_item_donestores it), soemit_failed'sfilter_map(|item| item.item_data.take())drops it entirely.
Net result: a harmony stream that dies after streaming N tokens emits response.failed with "output": [] (only previously-completed tool-call items survive), even though emitter.accumulated_text holds the delivered text. That's the same partial-output loss the PR is fixing on the other two paths.
Same applies to line 494. Either have the processor thread its message-item state into the emitter (set current_message_output_index/current_item_id/has_emitted_* when it allocates), or give emit_failed an explicit "open message item" parameter the harmony caller can supply.

🟡 Nit: reason is hardcoded to "backend_error" here, but convert_and_accumulate_stream returns StreamOutcome::Failed("stream_read_error", …) for transport read errors (line 1078) as well as parsed backend error frames (line 1108). Every MCP-loop read error is therefore booked as backend_error, so the two-value closed set the metric doc promises is only actually honoured on the non-MCP path.
Relatedly, the non-MCP classification at line 252 recovers the reason by string-comparing the error code (code == "stream_read_error"), which silently misattributes any upstream error frame that happens to carry code/type of "stream_read_error" — the code is attacker/backend-controlled, the reason shouldn't be derived from it.
Cheapest fix that covers both: make the reason part of the outcome rather than re-deriving it, e.g. Failed { code, message, reason: &'static str } (and the non-MCP failure tuple likewise), so each construction site names its own reason once.

🟡 Nit: smg_responses_stream_failures_total has no matching describe_counter! in the init block (lines 210-520), unlike its neighbours (smg_mcp_tool_iterations_total, smg_router_request_errors_total, …). Without it the exporter emits no # HELP/# TYPE for this series, so the counter shows up undocumented in /metrics and in any scrape-metadata-driven dashboard. Worth adding alongside the other MCP/router counters:
describe_counter!(

🔴 Important Persist the response returned by emit_failed.
emit_failed builds the terminal response sent to the client. This branch discards that response and persists accumulator.finalize_failed(...) instead.
If the failure occurs before a valid chunk, the accumulator persists an empty response ID, model, and timestamp. If a tool call started before the failure, the accumulator also retains its "in_progress" status. Persist the emit_failed result so storage matches the terminal client event.

709 - 712, Update the failure classification in convert_and_accumulate_stream so
body read failures are recorded with the regular stream path’s read-error
reason, while preserving backend_error for genuine backend failures.
A backend failure after the first byte of a /v1/responses stream never
became a terminal event. The regular path relayed the chat layer's
error frame verbatim and then emitted response.completed -- and
persisted the truncated response as completed; the MCP loop dropped the
error frame on the floor entirely and completed; harmony emitted a bare
error frame with no event name and no item closure. In every case the
client is told a truncated answer succeeded, and the failure lands in
no metric: the request was already 200 when the stream opened, so a
backend death after first byte is indistinguishable from success (or
from a client disconnect).
Give the lifecycle its missing terminals -- response.failed and
response.incomplete join the event enum -- and teach the shared emitter
emit_failed(): close the open message item with the standard done
ladder stamped "incomplete", then emit response.failed carrying a
typed {code, message} error and every item that produced output,
completed and incomplete alike; dropping the partials is exactly the
failure mode this event exists to fix. The regular path recognizes the
chat layer's error frame (and read errors) and terminal-fails instead
of completing, persisting the failed response with its partials -- what
the client was shown is what is stored. The MCP loop's accumulator now
returns a typed outcome, so an error frame aborts the loop into
emit_failed instead of vanishing. Harmony's pipeline and processing
failures route through the same contract.
A new smg_responses_stream_failures_total{model, reason} counter makes
these visible at all; reason is a closed set (backend_error |
read_error), never a raw upstream string.
The provider-proxying openai router path has the same defect class
(including overwriting an upstream response.failed with a synthetic
completed) and is left for a follow-up: separate subsystem, own
accumulator, and this change is already protocol + emitter + three
paths.

Review follow-ups on the mid-stream failure contract. A message item that had already completed normally was re-closed by emit_failed with an empty incomplete replacement, because the emitter never forgot the current message after a finish frame; completing an item now clears that state and the incomplete closer skips completed items. The Harmony processor keeps its message item in local state, so a failure on that path attached no partial text; it now registers the open item with the emitter, which already tracks the streamed text. emit_failed builds the typed failed response, emits its serialization and returns it, and the non-MCP path persists that instead of an accumulator reconstruction that lost the response identity when the failure preceded the first chunk. The failure-reason metric label is a typed enum decided where the failure is observed rather than recovered from an upstream error code, so MCP read failures are booked as read_error and an upstream frame cannot steer the label. The counter gains its describe_counter entry. Message items on the normal completion path now carry status "completed", which is part of the wire shape and what lets them round-trip into the typed persisted response; a typing failure during persistence is logged instead of silently dropping the item. 🔴 Important: emit_failed now returns the typed ResponsesResponse precisely so callers can persist what the client was shown — the regular path adopted that in this push (regular/responses/streaming.rs:255-266), but all four harmony failure branches still discard it and return without persisting.
Failure scenario: store is unset (defaults to true), harmony streams N tokens, then process_responses_iteration_stream returns Err. The client receives response.failed with id: resp_…, but nothing is written to response_storage, so the follow-up GET /v1/responses/resp_… 404s — while the success path two lines below (line 508-511) does persist. So a harmony request that fails mid-stream is indistinguishable from one that was never made, and any conversation the request was attached to keeps no record of the partial turn.
The fix is now a two-liner per branch:
let failed = emitter
.emit_failed("processing_error", &err_msg, None, tx)
.await;
persist_response_if_needed(
ctx.conversation_storage.clone(),
ctx.conversation_item_storage.clone(),

)

return;Same for lines 240, 267 and 467. (CodeRabbit raised the persistence gap on the MCP branch and marked it addressed in b13f7c4..cf2f9ab, but harmony/responses/streaming.rs is unchanged in this push — it is still open on all four sites.)

🟡 Nit: Persisting the emitter's response instead of accumulator.finalize_failed(...) buys the correct identity/echo, but it silently drops two kinds of partial output that only the accumulator sees on this path.
ResponseStreamEventEmitter::process_chunk handles delta.content and finish_reason only — it never looks at delta.reasoning_content or delta.tool_calls. StreamingResponseAccumulator::process_chunk accumulates both (reasoning_buffer, tool_calls, lines 336-384), and finalize() turns them into a reasoning item and function_call items for the completed record.
Failure scenario: a tool-calling request streams delta.tool_calls with 3 KB of arguments and no text content, then the chat layer emits an error frame. event_emitter has zero tracked items, so the persisted failed response is {status: "failed", error: {...}, output: []} — the accumulated call id/name/arguments are gone. The same request completing normally persists that function_call item. Ditto for reasoning-only prefixes: reasoning_buffer is discarded on failure, kept on success. The previous push's finalize_failed retained both as incomplete/in_progress.
Cheapest fix that keeps the "persist exactly what the client saw" property for the items the emitter owns while not throwing away the rest: append the accumulator's reasoning/tool-call items to the emitter-built response before persisting (or feed them through the emitter so they also appear in the response.failed payload).
Unrelated but visible right here: the completed branch below persists accumulator.finalize(), whose response_id is the upstream chunk id (self.response_id.clone_from(&chunk.id), line 326) — i.e. chatcmpl_…, not the resp_… the client saw in response.created. After this change the two terminals of the same function disagree on the stored id (🟣 pre-existing on the completed side, but now asymmetric).
| warn!( | ||
| output_index = item.output_index, | ||
| error = %e, | ||
| "Output item could not be typed for persistence; dropping it" |
There was a problem hiding this comment.
🟡 Nit: This warn (good idea — it did surface the missing message status) will fire on every healthy harmony response, because reasoning items can never type-check.
emit_reasoning_item (line ~890) stores {"id":…, "type":"reasoning", "summary":[], "content": <Option<String>>, "encrypted_content": null, "status": null}, but ResponseOutputItem::Reasoning.content is Vec<ResponseReasoningContent> with no #[serde(default)] (crates/protocols/src/responses.rs:2243). So "content": null (harmony, harmony/streaming.rs:821 passes None) and "content": "text" (regular MCP loop, regular/responses/streaming.rs:1013) both fail with "invalid type: null, expected a sequence".
Failure scenario: any gpt-oss/harmony streaming request that produces analysis output → emitter.finalize() at harmony/responses/streaming.rs:410/508 logs WARN Output item could not be typed for persistence; dropping it once per request in steady state, and the stored response silently loses the reasoning item (the drop itself is 🟣 pre-existing — .ok() hid it — but the log noise is new, and it will also drop the item from the new response.failed payload, which is now built from this typed path).
Fixing the producer clears both: build the item with the typed constructor the non-streaming accumulator already uses —
let item = serde_json::to_value(ResponseOutputItem::new_reasoning(
item_id.clone(),
vec![],
reasoning_content
.map(|text| vec![ResponseReasoningContent::ReasoningText { text }])
.unwrap_or_default(),
None,
))— which also stops emitting a spec-invalid content: null on the wire in output_item.added/.done.

Closing as superseded in substance. #2665 and #2674 landed the Two small pieces from this branch are not on main: a typed |
Description
Problem
A backend failure after the first byte of a
/v1/responsesstream never becomes a terminal event — on any gRPC-native path:response.completed— and persists the truncated response as completed.response.completedover truncated output with no error signal whatsoever.data:-only error frame — no SSE event name, no item closure, and open items are dropped from any subsequent output.response.failed/response.incompletedid not exist in the codebase — the lifecycle enum could not represent them — even though the wire model (ResponseStatus::Failed,error,incomplete_details) exists and non-streaming uses it correctly. And because the HTTP status was already 200 when the stream opened, these failures land inhttp_responses{status="200"}: a backend death after first byte is metrically indistinguishable from success — or from a client disconnect.Solution
One terminal-failure contract, applied to all three gRPC paths:
ResponseEvent::{Failed, Incomplete}join the lifecycle enum (crates/protocols/src/event_types.rs) withis_response_eventcoverage.emit_failed(code, message, usage, tx): it closes an open message item with the standard done ladder (output_text.done→content_part.done→output_item.done) stamped"status": "incomplete", then emitsresponse.failed(a named SSE event) whose response object carriesstatus: failed, a typed{code, message}error, and every item that produced output — completed and incomplete alike. Dropping the partials is exactly the failure mode this event exists to fix. It returns the response object so callers persist exactly what the client was shown.parse_error_frame) and read errors, terminal-fails instead of completing, and persists via a newfinalize_failed(statusfailed, error attached, partial items stampedincomplete) instead of storing a truncated "completed" response.convert_and_accumulate_streamreturns a typedStreamOutcome::{Complete, Failed}; an error frame aborts the loop intoemit_failed, with accumulatedmcp_list_tools/tool-call items riding along in the output.emit_failedinstead of the bare error frame.smg_responses_stream_failures_total{model, reason}— the only signal that a 200-established stream died on the backend.reasonis a closed set (backend_error|read_error), never a raw upstream string, keeping cardinality bounded.Deliberately out of scope (follow-ups, same defect class): the provider-proxying
openairouter path (separate subsystem and accumulator; it additionally overwrites an upstreamresponse.failedwith a syntheticcompleted), themax_tool_calls/max_iterationslimit semantics on harmony (behavior-design questions, not backend failures), and mock-worker mid-stream fault injection for e2e coverage.Changes
