Major · Functional Correctness · Heavy lift

Do not accept a stale remote HEAD as the default branch.

After a remote default changes from main to master, the local refs/remotes/origin/HEAD can still name main. resolveDefaultBranchName reads that local symbolic ref, and this return bypasses the candidate checks. A later launch can start from an obsolete local main or fail to create its worktree when main no longer resolves. Refresh or verify the remote default before persisting the base ref. (raw.githubusercontent.com)

Based on learnings, “refs/remotes/origin/HEAD ... is NOT updated by git fetch,” even after a remote default-branch rename.

Review comment at @apps/server/src/mcp/OrchestratorMcpService.ts at line 1809:
Update the `remoteDefault` early return so a stale local remote HEAD cannot
bypass the candidate checks; verify it against the current remote default or
existing candidates, and call `onRemote` only with a verified default branch.

Partly addressed in 130b62a: the remote default is now only used if its tracking ref exists, otherwise it falls back to main and master on the remote, so a renamed and pruned default branch picks up origin/master.

Not addressed: if the old tracking ref was not pruned, origin/HEAD and the ref both still look valid, so the task starts from the old branch. Tasks already did that before this PR. Detecting it needs a network call during schedule_task, so I left it out and listed it as a limitation in the PR description. Leaving this open for a maintainer to weigh in.

@tuanvumaihuynh The supplied code confirms the partial fix: resolveScheduledTaskBaseRef checks the remote tracking ref before selecting the recorded default. If that ref is missing, it checks remote main and master.

The remaining case is distinct: an unpruned tracking ref can resolve successfully even when it no longer names the remote’s default branch. The existence check does not establish freshness.

I agree with leaving this thread open for a maintainer’s scope decision. I am not requesting a network call in this PR. Would you like me to open a follow-up issue for stale-default detection?

## Durable intent and side effects

The [effect worker](../../apps/server/src/orchestration-v2/EffectWorker.ts) performs side effects
after intent has been recorded, then feeds results back into orchestration. A command acknowledgement
therefore means the intent committed, not that the provider, checkpoint, or other follow-up work
finished. Keep external I/O out of command decisions and the database transaction. Effects tied to
a lost provider process cannot simply replay; recovery retires them before admitting new work.
