fix(server): Codex questions the agent waits on put the thread in waiting for input - #26954
When a Codex thread asks the user a question and then stops to wait for the answer, it never enters the waiting-for-input state. The sidebar, the thread list, the mobile Live Activity and t3 thread show all keep reporting it as Working, and no waiting-for-input push is sent. A Claude question in the same project goes to waiting-for-input and pushes as expected.
How it was found: on an installed nightly (0.1.5-nightly.20261029.3867), a Codex thread on gpt-6-astra asked one structured question. The relay log showed statePhase going starting → running → completed and never waiting_for_input. The phone showed the thread as Working and got no notification. This happened in two separate runs.
Root cause
gpt-6-astra asks with request_user_input_async and then waits for the answer by calling Codex's interruptible clock.sleep tool, which ends with "Sleep interrupted by new input" when the answer arrives. The adapter records async questions with responseCapability: { type: "message" }, so the shell summary marks them blocking: false. Since #15430, a non-blocking question on a live run reads as Working everywhere. That is correct while the agent keeps working, but here the agent is idle until the user answers.
Change
Codex adapter: remembers the turn's async questions. When a sleep item starts it re-emits them with responseCapability: { type: "message", agentWaiting: true }, and when the sleep ends it clears the flag. It also clears the flag when Stop or a failure ends the turn before the sleep does, and ignores a repeated completion of a question it already tracks, since request ids are random and a repeat would add a second, unmarked question. Answer delivery is unchanged: still a message.
Contracts: adds the optional agentWaiting field to the message response capability. Older payloads decode unchanged.
Projection store: the shell summary treats an awaited question as blocking. The client presentation, sidebar status, agent awareness (Live Activity and push) and snooze already key off that flag, so web, desktop and mobile follow without client changes.
Event sink: the existing guard on provider writes for user_input requests now also drops a pending update for a request that is already answered or dismissed. Without this, the sleep ending after the answer would reopen the answered question.
Runtime recovery: a server restart or shutdown during the sleep loses the adapter that would clear the flag, so recovery clears it. The question stays pending and answerable, as async questions already do across restarts. Recovery's commit now runs the same in-transaction user-input guard, through a new commitCommand option, so an answer that lands during shutdown is not overwritten.
Resumed turns: a turn that resumes the same provider thread, such as a restart continuation, receives that thread's pending message questions. The Codex adapter seeds them into the turn, so sleeping on a question asked before the restart puts the thread back in Input. Run execution accepts updates only for those inherited request ids in that app thread.
Docs: a short paragraph in docs/internals/providers.md (protocol trap) and one sentence in docs/user/providers-codex.md.
Other providers need no change. ACP (Cursor, Grok, Antigravity, registry), OpenCode, Muse and Pi only create live questions, which are already blocking.
Scope and approval
This is a focused fix for an obvious bug, with no prior issue. A question the agent is idle waiting on is the same situation as any blocking question, but users get neither the waiting state nor the push. #15430 deliberately keeps async questions Working while the provider keeps working. This PR leaves that behaviour alone and only changes the case where Codex itself has stopped to wait.
Related but separate: #24914, and the open PRs #24922, #25155, #26804 and #24894, handle the other case, where the agent keeps working after asking. They add a marker while the status stays Working. This change touches only the server-side blocking signal for the sleeping case, so the two are complementary.
Verification
Tests, each written first and failing on main, extending existing cases:
CodexAdapterV2.test.ts, the async question replay now includes a sleep item and runs twice. When the sleep completes, the request is emitted as message, then message + agentWaiting: true at sleep start, then message at sleep end. When the turn is interrupted before the sleep completes, turn cleanup emits the final message. A third row restores a question from before a restart and expects only the awaited and cleared updates. Every row repeats the question's completion during the sleep and expects no extra request. On main only the first emission happens.
RunExecutionService.test.ts, provider event routing accepts an earlier attempt's request update only when the run inherited that request, and rejects the same update for another thread or another request id.
ProjectionStore.test.ts, the shell question table gets a row: an awaited async question is blocking: true in both the SQL and in-memory shell builders.
runtimeLayer.test.ts, the async-answer test: after the answer, a provider's pending repeat of the request leaves it resolved. It fails without the event sink change.
ProjectionRecovery.test.ts, with the real event sink and projections: shutdown recovery keeps an awaited question pending with agentWaiting cleared, and leaves a question answered between recovery's read and its commit resolved. The first fails without the recovery change and the second without the guard.
vp run check:changed against current main passes format, lint, knip, graphql, typecheck (36 packages) and 457 tests in the 10 test files beside the changes.
Manual check on the desktop dev build (vp run dev:desktop, isolated home), with a real Codex thread on gpt-6-astra asking one question:
Before (main): the thread shell reports running with a non-blocking pending request, so the awareness phase is running and the sidebar shows Working while the question is open.
After: the awareness phase goes running → waiting_for_input (run still live, request blocking: true) → back to running once the question is answered in the UI → completed. The sidebar shows Input. The persisted request stays resolved after the sleep ends. Codex's session log confirms request_user_input_async, then clock.sleep, then "Sleep interrupted by new input".
Before: the question is open, but the sidebar says Working.
After: the same question puts the thread in Input.
Not checked end to end:
Stop during the sleep, and a server restart during the sleep. Only the tests above cover them; the manual check covered the plain question and answer.
The push itself. A dev instance isn't linked to T3 Connect, so the relay skips publishing. I ran the relay's own shell → projectThreadAwarenessV2 projection against the dev database and saw waiting_for_input, but did not see a notification arrive on a phone.
The mobile app, the web client in a browser, and remote/relay/tunnel connections. They read the same blocking flag, but I did not run them.
Codex models without the async question tool. gpt-6-luna reported the tool as unavailable, so it never reaches this path.
t3 thread show still prints the run status (running), as it does for a Claude question, alongside Waiting for 1 answer(s).
Implemented with Claude Opus 5.5 through Claude Code, running in T3 Code.
…ting for input
Codex asks with request_user_input_async and then idles in the interruptible
clock.sleep tool until the answer arrives. T3 Code keeps async questions
non-blocking while their run is live, so these threads stayed "Working"
everywhere and no waiting-for-input push was sent.
The Codex adapter now marks the turn's async questions `agentWaiting` while a
sleep item runs and clears it when the sleep ends, and the thread shell treats
an awaited question as blocking. The event sink drops a provider's pending
repeat of a question that is already answered, so the sleep ending after an
answer cannot reopen it.

Macroscope's review found this PR approvable — Codex questions now show input-needed status while the agent waits, using the existing notification behavior. A transactional guard prevents subsequent pending updates from reopening answered questions; the change presents no obvious material risk.

The Codex adapter now marks async questions as awaited during sleep and clears that state when sleep completes or the turn ends. Projection and recovery logic use the state to classify requests, while the event sink filters repeated pending updates for settled requests.

Message response capabilities can include agentWaiting. The Codex adapter updates this state during sleep and turn finalization. The projection treats awaited message requests as blocking. Tests cover completed and interrupted turns and blocking requests.
The event sink can filter a pending update when the stored request is already settled. A regression test checks that the resolved request remains unchanged. Internal documentation describes this handling.
Recovery includes pending message requests marked agentWaiting in its updates and enables the pending-request guard. A recovery test checks requests that remain pending and requests resolved during the projection read.

Explanation
The description follows the repository template closely. It has Problem, Change, Scope and approval, and Verification sections, and each section has substantive content. The Problem section gives the observed behavior, reproduction context (a nightly build, a gpt-6-astra thread, and the relay log showing statePhase going from starting to running to completed), and the expected behavior. The Change section explains the fix across the Codex adapter, contracts, projection store, event sink, runtime recovery, resumed turns, and docs, and explains why the changes are needed for the same fix. The Scope and approval section explains why the change qualifies as a focused fix for an obvious bug without a prior issue, and it distinguishes this change from related open PRs. The Verification section lists the focused tests with their pass or fail behavior on main, the repo check results, and a manual desktop check with before and after evidence. It also states what was not checked end to end: stop during sleep, restart during sleep, push delivery, mobile, web, remote connections, and models without the async tool. The template comment asks for the model and harness that did the work, and the author provided that (Claude Opus 5.5 through Claude Code). Minor gaps: the title is not shown in the description body, and the scope section does not link to a triaged issue or maintainer approval comment, which the template allows only as an alternative to the obvious-bug explanation. The author did provide that explanation.

Stop or a failure can finalize a Codex turn before its sleep item completes.
The adapter then dropped its async questions without clearing `agentWaiting`,
so the unanswered question stayed blocking and a later turn would show Input
while the agent worked. Turn finalization now resets any awaited question, and
the reset skips questions that are already in the requested state.

A server restart or shutdown while Codex sleeps on an async question loses the
adapter that would clear `agentWaiting`. Recovery keeps message questions
pending, so the question stayed blocking and the thread showed Input while a
restart continuation or the next turn worked. Recovery now clears the flag and
leaves the question pending and answerable.

A runtime-request.respond already past the shutdown gate can answer a question
after recovery reads the thread. Recovery then committed its stale pending
payload over the answer. commitCommand now accepts the in-transaction user-input
guard the other event sink writes use, and recovery enables it. A commit whose
events the guard drops entirely still records its receipt.
The recovery test moves to the real event sink and projections, and answers a
question between recovery's read and its commit.

Preserve agentWaiting when rebuilding an async question.
When the transport delivers the same async item/completed notification after sleep starts, this branch creates a new request with responseCapability: { type: "message" }. The newer request can become the selected pending request, so awareness stops reporting waiting_for_input.
Carry the marker from the existing request into the emitted and stored request.

Confirmed Android input-needed push delivery while the Codex turn was still running on fdf693e516d44ffc6bf78b8483a9c3bd58b99753.
Environment: Play Store T3 2.2.0 (Android API 37), macOS 26.6.2 arm64, Codex CLI 0.166.0, gpt-6.1-sol. Isolated, unmodified PR checkout using T3 Connect activity publishing without a managed tunnel.
request_user_input_async followed by native clock.sleep: the phone received the input-needed push before the turn ended. A host snapshot after receipt confirmed the question was pending, agentWaiting: true, and completedAt: null. Relaying the answer through the server API completed the turn without reopening the question.
Repeated with one unanswered question and three consecutive 60-second sleeps, with the phone backgrounded and locked. The tester reported one input-needed alert and a turn-ended notification around completion; no duplicate input alerts were observed despite the host re-entering waiting between sleeps.
Scope: actual Android push receipt verified. Notification tap-through and answering from Android were not verified—the isolated environment was not paired on the phone.
Runtime request ids are random, so a repeated item/completed for an async
question the turn already tracks added a second pending question. That copy
was not marked awaited, so a sleeping turn could fall back to Working. The
adapter now ignores the repeat, including for questions restored after restart.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Addressed the outside-diff finding from @coderabbitai on CodexAdapterV2.ts (preserve agentWaiting when rebuilding an async question) in 7485737, with a different fix than the suggested one. Runtime request ids are random, so a repeated item/completed for the same async item adds a second pending question rather than replacing the first. Copying the flag onto that duplicate would keep the status right but leave two questions. The adapter now ignores a repeated completion of a question the turn already tracks, including questions restored after a restart. The async question replay test repeats the completion during the sleep in all three rows and expects no extra request.
Also merged current main (027d985). The only conflict was the test layer list, which now includes NodeCrypto.layer. vp run check:changed against main passes: format, lint, knip, graphql, typecheck (36 packages), 457 tests in 10 files.
@abhi12299 Thanks for the correction. I inspected 74857379aa and the replay assertions. Ignoring the repeated completion is the correct fix here. Copying agentWaiting onto a new request would still create a duplicate question.
The guard checks the native question ID before creating artifacts. It also covers questions restored into context.asyncQuestions. All three replay cases repeat the completion during sleep and assert no extra runtime-request update.
This addresses my outside-diff concern. I did not rerun the checks; the 457 passing tests are your reported results.

Problem
