Onboarding import ignores the name a user gave a Codex conversation and titles the thread from the first user message. In #24067, a chat renamed to "Renamed chat" in Codex arrived in T3 Code titled <external_codex_apps_open_page>{"page_id":null}</external_codex_apps_open_page>. Two commenters reproduced it on macOS and asked for saved Codex names to carry over.
Codex keeps those names in session_index.jsonl in the Codex home. Each rename appends a {"id", "thread_name", "updated_at"} line, and Codex reads from the end so the newest entry wins. The importer never reads that file.
Change
AgentSessionScanner now records which Codex home each transcript came from. Before it returns an importable Codex thread, it reads that home's session_index.jsonl and uses the newest nonempty thread_name for the session ID as the title. If no name is saved, the title falls back to the current derivation.
The read covers the last 1 MiB of the index (thousands of renames) and drops a partial first line.
The index is read at most once per home per project import, and only when that project has an importable Codex thread.
As in Codex's batch lookup (find_thread_names_by_ids), the newest nonempty name wins. A blank entry does not erase an earlier name.
A missing or unreadable index means no saved names. Message text and Claude titles are unchanged.
Scope and approval
This is the first suggestion in juliusmarminge's triage of #10513 (labelled accepted): "Prefer a Codex saved title if the transcript/session metadata has name / threadName." The rollout has no such field, so this PR reads the index file where Codex stores the name.
It also fixes the #24067 title complaint for chats with a saved name.
Choices for maintainers to confirm. These await maintainer review:
Reading Codex's session_index.jsonl directly, with a 1 MiB tail bound.
Saved names are not truncated, matching how imported Claude renames are handled today.
Threads already imported keep their old titles. Repairing them needs a separate re-import or migration path.
Not covered: unnamed chats starting with <external_codex_apps_open_page> still take that markup as their title; that needs a separate addition to #20278's setup-block list. The other problems in #24067 (image attachments, command and compaction markup, missing projects) are also out of scope.
Verification
Linux x64, run from apps/server:
vp test run src/project/AgentSessionScanner.test.ts src/project/AgentSessionImporter.test.ts
vp run typecheck
vp lint src/project/AgentSessionScanner.ts src/project/AgentSessionScanner.test.ts # from repo root
vp fmt --check src/project/AgentSessionScanner.ts src/project/AgentSessionScanner.test.ts
The new test's rollout starts with the reported markup; its index holds an older name, another session's name, the newest name and a blank entry. With the fix reverted, 1 test failed and 53 passed: the title was the markup. On the branch, all 57 tests in both files passed. Typecheck, lint and format passed.
UI check: one synthetic Codex home, imported through real onboarding with fresh state on both revisions, in the web app (headless Chromium) and the Linux desktop app. Its two rollouts and two-line index were written by hand in the SessionIndexEntry format from Codex's source. Main titles the renamed chat with the markup; the branch titles it "Renamed chat". The unnamed chat keeps its derived title on both. Debug builds of the iOS and Android apps, paired to the same servers after each import, list the same titles.
Not checked:
An index written by a real Codex desktop rename. The fixture was written by hand.
macOS and Windows.
Import started from the mobile app, which has no import flow.
Codex shadow homes, and indexes larger than 1 MiB.
Prior PRs

#14906 by debugrror rebuilt that work on V2. It was closed because it exceeded the contribution size limit, with a request for a smaller title-import fix. This PR implements only the saved-name lookup and leaves existing titles unchanged.
#20278 by only21mil skipped injected setup blocks when deriving Codex titles. This PR builds on it.
Evidence
Web, before: main titles the renamed Codex chat with the injected markup.
Web, after: the same import uses the saved Codex name "Renamed chat".
Linux desktop, before: the same markup title.
Linux desktop, after: the saved name, while the unnamed chat keeps its derived title.
iOS, before: the thread list shows the markup title from main's import.
iOS, after: the same import listed as "Renamed chat".
Android, before: the same markup title.
Android, after: "Renamed chat", with the unnamed chat's derived title unchanged.
The new test fails with the fix reverted, receiving the markup title.
The new test passes on the branch, along with the scanner and importer suites.

Codex records each rename in session_index.jsonl. Import ignored it and
titled threads from the first user message, so a renamed chat could
arrive titled with injected markup. Read a bounded tail of the index and
use the newest nonempty name, as Codex does.

The scanner reads saved names from the Codex session index and uses a matching name as the title for a recent imported thread. A test checks title selection when the index contains multiple names, another session’s name, and a blank name.

Summary
Read and decode saved Codex names apps/server/src/project/AgentSessionScanner.ts
The scanner decodes session-index entries and reads at most the final 1 MiB of the index. It skips a partial first record when reading from the middle of the file.
Apply saved names to recent threads apps/server/src/project/AgentSessionScanner.ts, apps/server/src/project/AgentSessionScanner.test.ts
Transcript candidates carry their provider home path. Recent-thread processing caches decoded names per home and uses a matching saved name as the imported title. The test checks that a later blank entry does not replace the saved title.

used repository playwright and /usr/bin/google-chrome against the real dev web client. added a project pointing at a real git repository, selected codex gpt-6.1-sol, and sent two real turns. codex 0.164.0 wrote their rollout files and returned ALPHA_READY and BETA_READY.
renamed alpha twice through codex app-server thread/name/set, first to Original saved alpha, then Renamed chat alpha. codex itself wrote session_index.jsonl. no hand-written transcripts, indexes, mocks, fixtures, or demo data.
imported the same provider sessions through onboarding into separate fresh t3 homes. main used alpha’s first user message as its title. the pr used Renamed chat alpha, proving the newest saved name wins.
beta had no saved name and retained the same derived title on both revisions. both imported prompts and assistant replies remained readable.
sent a follow-up from the imported alpha thread through the pr web client using codex gpt-6.1-sol; it returned ALPHA_RESUMED, retained its saved title, and persisted the reply.
blacksmith checks run this pass: all 57 scanner/importer tests passed; scoped lint had 0 errors and 0 warnings; both changed files passed format checking. existing github ci remains red: Lint reports an unrelated pre-existing long comment at apps/server/src/orchestration-v2/Orchestrator.ts:603, and the aggregate Check fails with it. that file is outside this pr’s diff.

blacksmith vp run check:changed passed: format, lint (0 warnings/errors), both knip checks, graphql, affected typechecks, and 55 tests. the scanner/importer suites also passed, 57 tests. two independent read-only reviews approved this exact head without findings. the old lint failure was an unrelated comment on the old base; no unrelated code was edited.
the earlier real-provider/browser evidence remains above. this fixup pass did not repeat client/native verification. waiting for checks on the new merge head before declaring readiness.

The reader makes one file.readAlloc(size - start) call. Effect 4.0.5 preserves a short underlying fs.read as a shorter buffer. A short read can omit newer session-index entries, so the imported thread can retain parsedThread.title.
Read until the requested range is complete or EOF. Add a test that supplies a short read. The dependency reference should target Effect 4.0.5, not Effect v3.
