Problem

Managed ChatGPT sign-in currently gives the Codex app-server a bearer token at launch through ACCESS_TOKEN. T3 can later renew and persist that ChatGPT credential, but an already-running managed Codex process keeps using the launch-time bearer. Active or later turns can therefore fail with 401 token_expired until the provider session is recreated.

Expected behavior: a running managed Codex session should be able to obtain the current T3-owned ChatGPT access token when Codex needs to retry authentication, without requiring an app or provider restart.

Change

  • Configure the managed model provider with Codex's native auth.command instead of env_key="ACCESS_TOKEN".
  • Add a session-scoped private local IPC token source. The hidden self-invoked T3 token command receives only that local endpoint; no bearer token is placed in the child environment or launch arguments.
  • Keep token renewal in the existing CodexChatGptAuth service, including its refresh serialization and persistence. Concurrent helper requests share one credential read/refresh.
  • Bind each running credential source to the ChatGPT profile selected at launch. Token rotation for the same profile continues; replacing the ChatGPT connection causes the old session to fail closed rather than receive another account's token.
  • Bound the local channel's request size, token size, connections and handshake time. Shutdown stops admission, destroys open sockets and drains already-admitted credential work before the endpoint is removed. Post-start listener errors close the credential channel instead of crashing the T3 server.
  • Keep the managed provider configuration profile-specific so recursive user config cannot inherit an ambient token-sharing env_key/auth configuration.
  • Update managed model-catalog lookup to request a fresh profile-bound credential directly from the auth service now that the runtime environment no longer contains ACCESS_TOKEN.
  • Avoid an auth/session lock inversion during credential replacement by stopping managed session scopes before taking the locks used to adopt the new credentials.

Ordinary Codex CLI authentication is unchanged.

This is an alternative implementation to #19895. It follows the same native auth.command direction, but keeps the credential source inside each managed runtime/session scope instead of introducing a process-wide broker/protocol service and the associated server/registry/orchestrator wiring.

Scope and approval

Fixes #17445 and also addresses the stale-live-session behavior reported in #16132.

The failure and intended behavior are established in the maintainer triage for #17445: the managed app-server receives one access token at launch and has no path to receive a renewed credential while it remains alive.

All changes serve that one bug. The internal token command and local IPC source are the transport needed for Codex's native credential callback; profile checks and model-catalog changes follow from removing the static bearer from the child environment.

Verification

Focused behavior tests on Windows:

vp test run \
  packages/shared/src/nodeRuntime.test.ts \
  apps/server/src/provider/CodexManagedRuntime.test.ts \
  apps/server/src/provider/CodexChatGptAuth.test.ts \
  apps/server/src/provider/CodexManagedTokenSource.test.ts \
  apps/server/src/provider/codexManagedTokenCommand.test.ts \
  apps/server/src/provider/Drivers/CodexDriver.test.ts

Result: 6 test files passed; 109 passed, 12 skipped, 0 failed.

The coverage includes managed auth.command configuration, removal of static token environment injection, profile replacement, credential rotation, concurrent token requests, private-channel framing/deadlines, listener failure handling, shutdown/admission behavior and cleanup.

Additional checks:

  • Changed-file lint: 0 warnings, 0 errors across all 11 changed files.
  • Changed-file format check: passed.
  • vp run --filter t3 typecheck: succeeded. It reports existing Effect suggestions in unrelated files, but no type errors.
  • git diff --check: clean before commit.

I also exercised the broader repository test surface locally on Windows. It is not globally green because a number of existing suites assume POSIX paths, Unix executable/shebang behavior, Unix sockets, LF-only output, or non-Windows file deletion semantics. I am not presenting those broad runs as passing verification for this PR; the focused changed behavior above is green.

Limits: no real-account hour-long expiry run and no packaged Electron/SEA end-to-end turn were performed. Human review of the local IPC/auth boundary is still appropriate.

Implemented and tested with GPT-5.6 Sol in ChatGPT using the MoonDesk/T3 Code workspace connector.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR replaces the default managed Codex token flow with a new session-scoped IPC/auth-command mechanism spanning credential refresh, process startup, shutdown, and account replacement. The authentication-sensitive change is substantial, includes new diagnostic suppressions, and has an unresolved high-severity logout race requiring human review.

📝 Walkthrough

Managed Codex now obtains ChatGPT access tokens through a profile-bound local IPC command. The runtime configures that command for the selected profile and removes access-token environment overrides. Credential access, token delivery, runtime setup, and shutdown behavior are covered by tests.

Changes: Managed Codex token delivery

Layer / File(s) Summary

Credential access can require a matching profile key. Credential adoption accepts an optional prepare effect that runs before session stopping and credential updates. Tests cover profile import and failed pre-stop validation.

Merge Risk: 🟡 Moderate · up to 99f21

Managed Codex now fetches tokens through a profile-bound local channel. One earlier concern about credential validation blocking later credential operations remains unresolved and should be checked before merge.

Security architecture risk: 🔵 Low · up to 99f21

The change reduces token exposure at launch and binds subsequent credential requests to the selected account. A low-severity inherited-environment exposure remains, but comparison shows that behavior predates this change. Packaged execution and platform-specific access controls have not been validated here.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — Each live endpoint releases access tokens for its launch-selected ChatGPT profile. Access relies on local operating-system permissions, not per-child authentication, so the channel does not establish isolation between otherwise authorized same-user processes. No remote listener or new cloud privilege is introduced by this transport.

Security Findings and Attack Paths

  • observed — The retained low-severity finding concerns inherited managed-auth environment values. Provider configuration can supply arbitrary valid variable names, and the runtime does not remove T3CODE_MANAGED_CODEX_AUTH_URL or T3CODE_MANAGED_CODEX_AUTH_SECRET. The base already propagated these values, while the head supplies its new token channel through arguments rather than those variables. The supported exposure therefore predates this PR; increased reachability or authority was not established.

Trust Boundaries and Controls

  • observed — Local request data is restricted to the fixed token request, with a 64-byte request limit, five-second handshake deadline, and 32-connection limit. Tokens are bounded to 64 KiB. Credential access rejects profile replacement before renewal, and the listener checks profile identity again before delivery.

Resilience and Maintainability Implications

  • observed — Shutdown stops admission, destroys open sockets, and drains admitted credential work before removing the endpoint. Disconnected clients do not cancel an admitted renewal midway through persistence. Post-start listener errors close the channel instead of propagating an unhandled server error.

Hardening Proposals

  • proposed — Validate cross-user denial on supported Windows runtimes before treating default named-pipe permissions as equivalent to the explicit Unix protections. This is a platform-boundary validation proposal, not an observed authorization failure.

Explanation

The description includes all required sections: Problem, Change, Scope and approval, and Verification. It explains the issue, implementation, linked issues, focused test results, limitations, and agent details.

Full details: Linked Issues check

Explanation

The PR meets the coding requirements for #17445. Managed Codex no longer receives a launch-time ACCESS_TOKEN; it uses profile-specific auth.command access through a session-scoped local token source. The source reads the current credential from CodexChatGptAuth, including refresh and persistence, and rejects credentials from a replaced ChatGPT profile. Focused tests cover token rotation, concurrent reads, refresh persistence, IPC limits, shutdown, and runtime configuration. The reported lack of a real-account expiry run does not show a coding requirement failure.

Full details: Out of Scope Changes check

Explanation

The changes stay within #17445. The token command, local IPC source, profile binding, managed runtime configuration, model-catalog lookup, and credential lock-order changes support renewal of credentials during an active managed Codex session. The provider-core changes support safe credential replacement. The added tests verify these paths, and ordinary Codex CLI authentication remains unchanged. No unrelated product change is demonstrated.

🩺 Stability & Availability | 🟠 Major | 🏗️ Heavy lift

Allow cancellation while prepare waits.

If validation remains pending, Effect.uninterruptible prevents cancellation of adoptCredentials. The controller stays in "stopping" and rejects logout and new sign-ins until validation settles. The ChatGPT caller uses prepare for discovery and identity verification, so a stalled request can leave the connection unusable. Make prepare interruptible and reset operation on interruption; keep only the state transitions protected.

The new provider-auth regression specifically covers a concurrent logout while transferred credentials are in the pre-stop validation phase. Invalid transferred profiles also assert that stopSessions is not invoked.

Seed the managed authentication variables in this test.

CodexManagedRuntime.ts copies options.environment into the Codex child environment but does not remove T3CODE_MANAGED_CODEX_AUTH_URL or T3CODE_MANAGED_CODEX_AUTH_SECRET. The current fixture does not supply either variable, so these assertions do not detect leakage when launch input contains them.

Also hitting this with managed ChatGPT sign-in on Linux, T3 0.1.2-nightly.20261021.3457 and Codex 0.162.1. T3's refresh succeeded, but the existing shared app-server continued returning 401 token_expired; recreating the session restored requests.

Redacted observations and reproduction steps, shared once on the alternative fix stack to avoid duplicating the evidence. This PR's native auth.command direction addresses the mechanism we observed. We have not tested this branch.
