fix: composer picks up new project skills without a server restart

A user reported that project skills (.claude/skills, .agents/skills) didn't appear in the / menu of a new thread until they sent a throwaway first message. Each workspace's skill scan was cached until the server restarted. The composer asked for it once per cwd, and the server answered every later request from that cache. A skill added to the project after the first scan stayed invisible until a new cwd forced a scan. Creating the thread's worktree is one such new cwd, which is why sending "hi" made the skill appear.

Workspace scans now expire after five minutes (PROVIDER_WORKSPACE_SNAPSHOT_TTL_MS):

• Server: refreshWorkspaceSnapshot returns a cached snapshot only while it is younger than the TTL. Each scan is stamped with its own start time. Before, it inherited the provider health check's checkedAt, which could already be minutes old.
• Web and mobile composers: they request a rescan when the cwd's snapshot is stale, at most once per TTL per scope. That cap means a client clock ahead of the server's can't cause a loop of rescans.

Within the window nothing changes: repeat composers reuse the cached scan, and "Restart agent session" still forces a fresh one immediately. A stale scan costs one rescan the next time a composer opens for that cwd. That is a file read for Claude, Cursor, Grok and Antigravity skills, plus the cached init probe where a provider has one. Nothing scans in the background.

Not addressed here: a "New worktree" draft still lists skills from the project's main checkout rather than the base branch it will check out.

Verified with providerSkills.test.ts (TTL boundaries), ProviderRegistry.test.ts (an expired scan is redone, a current one isn't), and typecheck of contracts, client-runtime, server, web and mobile.
