Code Review
This pull request introduces a metadata-digest fingerprinting mechanism to resolve deadlocks on metadata-only fix requests, allowing the review cascade to re-arm when PR metadata (body, labels, or linked issues) changes without requiring a new commit. The feedback recommends improving shell script robustness and portability by implementing a fallback chain for sha256sum on macOS, replacing head -1 pipes with Bash parameter expansion to avoid SIGPIPE crashes under pipefail, and asserting explicit exit codes instead of using ! grep in BATS tests to prevent false passes.

Security Fix — D Rating Addressed Fixed in : Improved the fallback hash function to ensure it always produces valid 16-character hexadecimal output. The previous fallback used on the canonical JSON, which produced non-hex characters and could fail digest validation. Now uses 4294967295 0 converted to hex format, ensuring the metadata change detection mechanism works correctly even on systems without standard hash commands (sha256sum, shasum, md5sum, md5). |

Replace cut pipes with awk substr() to extract hash digests — provides explicit error handling and ensures all code paths output valid 16-char hex strings. Avoids SIGPIPE issues under pipefail and matches the robustness pattern already used in the cksum fallback. Each branch now has `|| printf '%016x\n' 0` fallback to guarantee valid output format even if hash commands fail unexpectedly. Co-Authored-By: Claude Haiku 4.5 <noreply@anthropic.com>
Remove md5sum/md5 fallback branches from compute_pr_metadata_digest() to fix SonarCloud Quality Gate failure (S4790: weak hash algorithm). Since sha256sum and shasum -a 256 already cover all realistic CI runners (GNU/Linux and macOS/BSD respectively), the MD5 fallbacks are unnecessary. The digest is only used for non-cryptographic change detection, not security-sensitive purposes. Co-Authored-By: Claude Haiku 4.5 <noreply@anthropic.com>

Summary
Implements the metadata-digest re-arm for issue #1551: metadata-only fix-requests now stamp a 16-hex digest of (PR body + closingIssuesReferences + labels) into the review marker, and review-one-pr.sh re-arms at the same head SHA when the digest no longer matches. Approvals and code-change fix-requests carry no meta= and keep commit-only re-arm. All five acceptance criteria are addressed with dedicated bats coverage; all CI checks are green.
Linked issue analysis
Closes #1551. AC1 (metadata change re-arms without a new commit): implemented via the meta= stamp in scripts/post-pr-review.sh and the digest comparison in scripts/review-one-pr.sh. AC2 (digest over body/closing-refs/labels): scripts/lib/pr-metadata-digest.sh canonicalizes with sorted refs/labels via jq -cS. AC3 (no churn vector): meta= is stamped only when the reviewer sets metadata_only=true on an escalate verdict; marker_meta_digest is anchored to the real fix-request marker (header + adjacent decision=fix-requested comment, whitespace-delimited attributes, exactly 16 hex), and boundary-guard tests cover stray text, run-on names (notmeta=), run-on decisions (fix-requestedness), and oversized values. Cycle cap stays authoritative. AC4 (diagnosable no-op line + footer): both re-arm wordings implemented for marker-with-meta and commit-only paths. AC5 (regression test): tests/test_review_one_pr_metadata_rearm.bats drives review-one-pr.sh end-to-end for re-arm, matching-digest no-op, and approval-marker no-op.
Findings
No blocking findings. Non-blocking observations: (1) The digest includes the PR body, which summary bots (e.g. CodeRabbit) edit — a bot body-edit after a metadata-only fix-request will trigger one same-SHA re-review; acceptable since meta= is scoped to metadata-only fix-requests and the cycle cap bounds churn. (2) The cksum fallback in compute_pr_metadata_digest is non-cryptographic, but it is a last-resort path for runners without sha256sum/shasum and the digest is used for change detection, not integrity. (3) The prior SonarCloud quality-gate failures on earlier commits were resolved; the gate passes at this head. Secret scan: gitleaks CI check passed; the run_secret_scanning MCP tool was not available in this environment (noted, not a failure). Diff contains no secrets, no auth/crypto surface, no injection patterns — all new shell honors set-o-pipefail concerns explicitly.
CI status
All required checks green at 67957d8 (shellcheck, bats, unit-tests, CodeQL actions+python, SonarCloud quality gate, gitleaks secret scan, agent-shield, actionlint, all validate-* gates). The cancelled dev-lead dispatch/ci-relay/resume entries are agent-orchestration runs, not code checks. New bats files are registered in .github/workflows/lint.yml.
