…arget An options object with url, filename, hostname or host is a complete connection target. The DATABASE_URL family of variables no longer supplies the adapter, the URL or the sslmode for such an object. Fixes the remaining half of #22147: an explicit target could still become a sqlite or mysql connection, or get sslmode=require forced on, from an unrelated variable in the shell.

Nothing blocking. The comments below are optional suggestions. There is no need to push a fix for them before merging.
Beyond the inline finding, I checked two adjacent concerns: the !! truthiness gate in hasConnectionTarget matches the downstream reads at lines 1709-1722 (which also skip empty-string url/filename), so { url: "" } falling back to env is consistent; and the test file's beforeEach already clears every SQL_ENV_VARS entry the new tests set, so no state leaks between cases.
Extended reasoning...
The inline finding covers the path omission, which the PR author states is deliberate and pins with a test — that's a design call a human should weigh. Separately I verified the empty-string edge of the new truthiness check aligns with how the resolver already treats empty url/filename values further down, and confirmed the existing beforeEach env reset covers all vars the new tests mutate. Nothing else surfaced.

LGTM — thanks for adding path to hasConnectionTarget and pinning it with the unix-socket test.
Checked that the env-URL gate now covers all five target-naming keys (url/filename/hostname/host/path) and that per-field env vars (PGHOST etc.) are untouched since only the getConnectionDetailsFromEnvironment call is gated. The new tests cover both the positive matrix (each *_URL env var × url/field forms, TLS_* not forcing sslmode, aliases) and the negative contract (options without a target — {port}, {max, username}, {adapter} — still fall back to env). The existing beforeEach env snapshot/restore in this file isolates the new tests' process.env mutations.
Extended reasoning...
Overview
This PR gates the getConnectionDetailsFromEnvironment(...) call in src/js/internal/sql/shared.ts on a new hasConnectionTarget(options) predicate, so DATABASE_URL/*_URL/TLS_* env vars are consulted only when the options object does not already name a connection target. The runtime change is 6 lines; the rest is 11 new test cases in the existing test/js/sql/adapter-env-var-precedence.test.ts and a docs paragraph. Since the previous review, commit 5abf479 added path to the predicate and a test pinning that a unix-socket path counts as a target, addressing the one issue I raised.
Security risks
None introduced. The change tightens precedence so explicit programmatic configuration beats ambient environment — the safer direction. It does not touch TLS handling itself (only stops TLS_* env var names from forcing sslmode=require onto an explicitly-configured target), does not touch credential handling, and does not add any new env-var reads.
Level of scrutiny
Low-to-moderate. The runtime change is a single conditional gate around an existing call; the predicate is a plain truthiness check over five well-known option keys. The behavior it produces matches what docs/runtime/sql.mdx already described. The variant matrix in the tests is thorough: every env-URL name, both option shapes (url string vs. field form), the host alias, filename, unix-socket path, the TLS_* names, and the negative cases where env fallback must still apply ({port}, {max, username}, {adapter} alone).
Other factors
The predicate uses truthiness (!!(o.url || ...)) rather than presence checks, so an empty-string url/hostname/filename is treated as "no target" and still falls back to env. That matches the surrounding ||-chain style throughout shared.ts and an empty string is not a usable target for the network adapters, so I don't consider it blocking. No CODEOWNERS entry covers these paths. Bug hunt ran to dry_streak with no findings.

LGTM — the earlier note about path is addressed (it now counts as a connection target, with a test), and the credentials-leak test added since covers the remaining hazard.
What was reviewed: the hasConnectionTarget gate in shared.ts — checked all five keys against the fields the resolver later reads (url, filename, hostname/host, path); a lone port/adapter still falls through to env, pinned by tests. The beforeEach/afterAll in the test file already snapshot and restore the full process.env, so the new USER/PASSWORD/PGPASSWORD writes don't leak. Docs paragraph matches the implemented key list.
Extended reasoning...
Overview
A 6-line guard in src/js/internal/sql/shared.ts stops parseConnectionDetailsFromOptionsOrEnvironment from reading DATABASE_URL/*_URL/TLS_* env connection strings when the caller's options object already names where to connect (url, filename, hostname, host, or path). This fixes env vars hijacking the adapter, target host, and sslmode of an explicit new SQL({ ... }) call. A docs paragraph and 13 new test cases in the existing adapter-env-var-precedence.test.ts accompany it.
Security risks
The change reduces a real hazard: environment credentials no longer get sent to a programmatically-named host, and an ambient SQLITE_URL can no longer silently redirect a Postgres client to a local file. No new attack surface is added — the guard only narrows when env is consulted. The truthiness check treats empty-string url/hostname as "no target", which is the sensible fallback (an empty string is not a usable target).
Level of scrutiny
Moderate. It is a behavior change to precedence rules, but the new behavior is the one the docs already implied ("explicit beats ambient"), and the PR pins the one intentional regression ({ url: process.env.TLS_DATABASE_URL } no longer inherits sslmode from the variable name) with a dedicated test. My earlier inline concern — unix-socket path being omitted — was fixed in a follow-up commit and is now covered by both the helper and a test.
Other factors
Since my last review only a test commit landed (env credentials not leaking to an explicitly-named host, and per-field PG* vars still filling gaps). The file's existing beforeEach/afterAll hooks snapshot and restore all of process.env, so the new env writes are hermetic. No CODEOWNERS entry covers these paths. No outstanding third-party changes-requested reviews.

{ adapter: "sqlite", url } used to ignore url and open the environment
URL or :memory:. With the environment lookup now gated on the options
naming a target, url must be honored here as well, so it opens the same
database that { url } alone does. filename still wins over url.
…explicit-config-precedence
…nsequence A sqlite-looking filename already wins over the environment URL, and a bare filename is not recognised as SQLite at all (separate bug), so the filename clause changed nothing that a test could pin. The check now lists url, hostname, host and path. The docs say that an explicit target also loses the sslmode=require that a TLS_* variable implies, and how to ask for TLS explicitly. The test pins that the options form now matches the string form, which never took the sslmode from the variable name, and that tls: true requires TLS.
