Batch C1 of feature 008 (Quality Debt to Zero, specs/008-quality-debt-zero/): the 27 cognitive-complexity, function-length and parameter-count findings in the observer and worker modules. 17 SonarCloud rows (16 S3776, 1 S107) and 10 Codacy rows (9 Lizard_nloc-medium, 1 Lizard_parameter-count-medium).

What changed

Extraction-only refactors under research rail R7 in src/observer/{catalog,classify,contract,fallback,llm,request}.ts and src/worker/{batches,observe,purge}.ts: a single-exit block becomes a named function, nothing is reordered, no export is added or removed, and a helper taking more than seven parameters takes an options object instead.

The largest is runObserve, 264 lines of code and cognitive complexity 140 on main. providerCall and processBatch took 9 and 10 parameters; both now take one options object, which is what closes the S107 and Lizard_parameter-count-medium rows.

processBatch needed a third pass. Two delegated passes left it at cognitive complexity 19 against a bound of 15, so this session finished it: the record-and-retry block became settleProviderOutcome and retryOnLanguageMismatch, each returning either the result the caller must return or the outcome to carry on with, and the three functions nested inside processBatch were hoisted to module level. The order of the calls and of every side effect is unchanged; a failed provider call still falls through to the caller's own fallback branch, the retry still records its own result before the second language check, and only a second mismatch marks the session degraded.

What the review changed

The delegated pass had not extracted runObserve but converted it: the worker's main loop became a generator driven by a hand-rolled trampoline, with every await replaced by a yield. R7 allows extraction because extraction cannot change control flow; a coroutine can, and a generator that is not driven to completion never runs its finally, so the heartbeat interval and the open database would leak. The sixteen generators are plain async functions again and runObserve awaits observeLifecycle.

The same review found, and this PR fixes: a let exit!: number that traded a compiler proof for an assertion; a boolean helper that lost the narrowing of ctx.preset and produced two as PresetName casts; two functions returning untagged unions where the falsy values meant opposite things (CallOutcome | null | undefined, and the lease token or the process exit code); a predicate named for the opposite of what it returns; five mutable out-parameters; a lookup-shaped name on a function that writes session state; four single-use wrappers that only renamed an expression; and ~30 lines of whole-block re-indentation that had nothing to do with the refactor.

How the numbers were checked

  • Length and parameters: pipx run lizard -l typescript -T nloc=50 -T parameter_count=8 -w over the batch's files, which is the tool Codacy runs. No function is over 50 NLOC or takes more than 8 parameters. The two remaining warnings are cyclomatic complexity, which Codacy does not gate on here.
  • Cognitive complexity: a local eslint-plugin-sonarjs run at threshold 12 against SonarCloud's bound of 15. On main it reproduces SonarCloud's own numbers for these files exactly (140, 62/61, 47, 24, 21, 20, 16). At this head it reports no warning in any of the batch's files.

Resource check (T032)

Base bb30a5bb in ~/projects/free-mem and the candidate in its own checkout, built separately and measured back to back on the same quiet machine between 18:27 and 18:47 JST.

node dist/oboete.mjs fixture replay test/fixtures/events-1000.jsonl --json:

measure base candidate delta

session start, pending max 1186.9 ms 1184.1 ms -0.2 %
worker peak RSS 116456 kB 126776 kB +8.9 %

Every structural verdict is identical on both sides: hooks 1143 of 1143 exit 0, lifecycle fork / resume / compact / clear all pass, SC-005 0, SC-010 0 with raw_events.id 1322 against 1051 lines, directives 0. The worker's peak RSS is 8.9 % above base, inside the 15 % band and well under the 150 MB bound; it is the one number this batch moves.

Both runs exit 1 for the same reason: SC-009 recall is ja 4/20, en 3/20, 7/40 overall against a bound of 90 %. That bound is the M2 embedding target and it is red on main too.

node scripts/measure-cold-start.mjs, three rounds a side:

scenario base p50 (median of 3) candidate p50 (median of 3) delta base max candidate max
--version 50.5 ms 50.0 ms -1.0 % 61.5 ms 56.9 ms

hook small, DB absent (spool) 162.9 ms 172.4 ms +5.8 % 170.8 ms 187.4 ms

Every median is within 6 %, every maximum is under the 300 ms budget and within 15 % of base.

Gate

  • tsc and eslint clean; npm test 867 unit and 171 script tests pass, exit 0.
  • No hook budget, deadline, or wire format changed (specs/007/contracts).

Ledger rows for the 27 ids are appended in this PR (docs/evidence/quality-debt-2026-09/ledger.json).

…ass left open

Codex reduced catalog.ts, llm.ts and batches.ts in a second pass. The
remaining one, processBatch in src/worker/observe.ts, is done here after
two Codex passes left it at 19 against a bound of 15: the record-and-
retry block becomes settleProviderOutcome and retryOnLanguageMismatch,
which return either the result the caller must return or the outcome to
carry on with, and the three functions nested inside processBatch are
hoisted. The order of the calls and of every side effect is unchanged.

Also drops a stale doc block the first pass stacked on stripPartial.

Local eslint-plugin-sonarjs at threshold 12 now reports no
cognitive-complexity warning in the batch's files, and lizard reports no
function over 50 NLOC or 8 parameters.

Reviewer's Guide

This extraction-only refactor addresses 27 observer/worker complexity, function-length, and parameter-count findings by introducing focused helpers, options-object APIs, and a generator-driven worker lifecycle while preserving call order, side effects, contracts, wire formats, lease/deadline behavior, and exit semantics; the stated static-analysis and test gates pass.

…ndings

An independent review of #165 found that runObserve had not been
extracted but converted: the worker's main loop became a generator
driven by a hand-rolled trampoline, with every await replaced by a
yield. Research rail R7 allows extraction precisely because it cannot
change control flow; a coroutine can, and a generator that is not driven
to completion never runs its finally, so the heartbeat interval and the
open database would leak. The sixteen generators are plain async
functions again, observeResult and the driver loop are gone, and
runObserve awaits observeLifecycle.

Also from that review:

- `let exit!: number` traded a compiler proof for an assertion. It is
`number | undefined` again with an explicit check before the first
read, so a future path that does not assign fails loudly instead of
reporting undefined as the exit code.
- providerConfigurationMissing returned a boolean and lost the narrowing
the inline check had, which is why two `as PresetName` casts appeared.
It is a type predicate now and both casts are gone.
- providerTextFailure returned `CallOutcome | null | undefined`, two
falsy values with opposite meanings. It returns a tagged TextCheck.
- claimObserveLease returned `string | number`, the lease token or the
process exit code. It returns a tagged LeaseClaim.
- hasFallbackExitReason was named for the opposite of what it returns.
- collectObserverEvents and collectNearbyMemories took five mutable
out-parameters between them; they return their results and the caller
concatenates the dropped rows in the order the single pass produced.
- existingSessionSummary also writes session state; it is
  finishWithExistingSummary.
- Four single-use wrappers that only renamed an expression are inlined,
  as is providerApplyInput.
- The whole-block re-indentation of CallOutcome, ERROR_OUTCOME_ROWS and
FallbackEvent is reverted; it was formatter noise in a diff whose
  reviewability is the point.
- The heartbeat's `token as string` is gone; the `db` cast stays with
  the reason it is needed.

emptyCounts keeps observeLifecycle under the length bound after the
async conversion.

require, and what all three batches were actually compared against, is that
every structural verdict is green and identical on both sides: hooks, the
lifecycle checks, raw_events against lines, duplicates, secrets, directives.
The clause now says that, so the tick is not standing on a comparison nobody
made.

…e the Sonar burst

Deleting `applyCodacy`'s pre-read last commit was right — Codacy's search reports only
the current analysed commit, so absence never proved an ignore. But the pre-read also
carried the only path that let a run continue past an id the service will not accept, so
one bad id aborted all 27 remaining rows. `applySonar`, rewritten in this same pull
request, had been given the opposite contract, which is how the asymmetry showed up: one
service collects refusals and finishes, the other stops dead.

`applyCodacy` now mirrors it. A 404 means the service no longer holds that issue record,
so the row is refused by name and the run continues; an auth or server error still aborts
immediately, because continuing would print a refusal line for every remaining row and
bury the real cause. One error at the end names the refused count and ids. `request`
attaches the status to the error it throws so the two cases can be told apart.

The `invalid` group added last commit wrapped too much. `liveKey` ran before the id-level
classifications, so a finding whose comparison field could not be read was recorded as
invalid and never tested against the inventory — even though the `uncovered` test reads
only the id. A live Codacy finding with `filePath: null`, or a Sonar project-level
component, was therefore missing from `uncovered` until someone fixed the field and
re-ran, and the committed test asserted that drop. The id-level verdicts now happen
outside the try and only the triple half is guarded. `checkLive` stays at cognitive
complexity 12.

`sonarIssueStates` sent its chunks of 100 with no spacing, and the first `do_transition`
followed the last GET with no gap, against an endpoint the rest of the file deliberately
spaces at 200 ms — and the suite already has a 429 case, so a regression would have
surfaced as a flaky HTTP failure rather than a missing-pause assertion. One module
counter now spaces every request of a run, which also retires `applySonarCalls`'s
`firstCall` parameter: the spacing no longer depends on the caller reassigning a returned
boolean, and the function drops to five parameters. An earlier round kept that round trip
on taste with no defect attached; this round supplies the defect, so the counter wins.

`checkLive` was also the only ledger reader that trusted rows without `validateInventory`,
so two rows for one id — one confirmed `fixed`, one `open` — could report a live finding
as contradicted on the strength of a row `--check` calls a duplicate. It validates now,
like the other three readers.

Four numbers in the record disagreed with the measurement, all re-derived from the ledger
and a live `--check-live` run:

- the closing table said Codacy `28 (25 confirmed, 3 still open)`; of the 28 inventory ids
among the 240 live findings, 23 are `resolved` and confirmed, 4 are `fixed` and
confirmed and 1 is `excluded` and unconfirmed, so it reads 27 and 1 — which is also
  what `--check` prints eight rows below;
- "Three of the Codacy ones are inventory ids whose rows are confirmed `fixed`" was four,
  and only two were named: `77635429…` (#166), `188840d7…` (#165), `6d0b78f8…` (#185) and
  `7d9b63ea…` (#185);
- "All 27 `resolved` Codacy rows have HTTP 204 receipts" understated the population, which
  is 29: 27 from this batch's calls and 2 from batch D's;
- the documented pre-merge lizard command omitted the changed file nearest the bound,
`quality-debt-record-services.test.mjs` at 489 NLOC against 500, while listing every
other one. It now measures all seven. That file is 491 after this commit's tests, so the
check it was missing from is the one that will catch it first.

The record claimed the `contradicted` group means this shape "cannot pass unnoticed
again", but nothing runs it: no CI step, no cron, no npm script, and the next sentence
correctly explains the mode must stay manual while 460 findings sit outside the inventory.
The claim is now what is true — the detector exists and the round that owns those findings
runs it — and the scheduled reader is filed as #223, blocked on #207. Adding the gate here
would be a new gate this batch cannot make pass.

Declined: a diagnostic for SonarCloud dropping `resolution` while keeping `status`. The
live API still returns both deprecated fields, the behaviour is fail-closed, and writing a
message for a removal that has not happened is speculation — the same ground the
`issueStatus` fallback was declined on.

236 tests pass, ESLint is clean, lizard reports no threshold exceeded across all seven
scripts, `--check --planned` exits 0, the record regenerates byte-identically, and
markdownlint reports 0 issues.

The delta review of 9e6ce79..879c465 verified every number in that commit and
then found eleven semantic defects, two of them blocking. Both blocking ones
came from the same root: the completion criterion had been written out in five
places while the exception that two rows depend on lived in a sixth that none
of them referenced.

FR-002 is now the only place the criterion is stated in full, and it defines
"confirmed" by pointing at the data model's field: an absence observed at a
named analysis, a service action receipt (Codacy `PATCH`, Sonar
resolution/comment), or, for regrowth handed to #207 whose absence was never
observed, the R11 before/after measurement tied to the earlier closing SHA.
SC-001 and SC-002 measure FR-002 instead of restating it, research R3 points at
it as the prerequisite it is, and the round 7 audit says outright that FR-002 is
authoritative and the audit only summarises it. Before this, FR-002 required
every one of the 745 rows to be "closed on `main` or confirmed against a named
analysis", which two rows cannot satisfy: `6d0b78f8…` and `7d9b63ea…` are live
at `33f8c382` and their confirmation is a measurement, not an analysis id. The
criterion was unsatisfiable as written, and adding the carve-out correctly would
have meant editing five documents.

The paragraph explaining Codacy's silence said three things that are not true.
It claimed `fixed`/`excluded` states unconditionally mean the named analysis did
not report the finding; that is what they mean only when the absence was
observed. It claimed `verifyCodacyCommit` runs "before any row is stamped",
which is false for the two rows stamped by hand: `isConfirmed` is a non-blank
string test and `confirmLedger` filters already-confirmed rows out before
verification, so a hand-recorded measurement never reaches it. And it implied
that any row whose finding regrows carries the R11 measurement, which would
have a reader reject two correctly-stamped rows. The four confirmed `fixed`
Codacy rows in `contradicted` split two ways, and the paragraph now names both
pairs: `188840d7…` (#165) and `77635429…` (#166) keep their observed-absence
receipt at `67242108`, because later regrowth does not erase what that analysis
observed; `6d0b78f8…` and `7d9b63ea…` (both #185) use the permitted measurement
because the current-commit search already reported them regrown.

The rest are one-clause corrections. The `checkLive` comment now says a triple
claims the rule no longer fires only when every inventory row in it is confirmed
`fixed`/`excluded`, matching `liveClaims`' AND-fold and what quickstart.md
already stated; the previous wording dropped that qualifier. The `history`
requirement says plainly that `--check` enforces neither a missing entry nor a
malformed one and that the reader is filed as #223, rather than stating a MUST
nothing reads. The 2026-09-09 inventory addition is credited to T030a, which the
record has always said, not to T036, which is the seam-table task and adds no
rows; the wrong attribution had been copied into four more places, and the
additions now read T030a 14 plus T043e 24. The `file`, `line` rule names an
export commit for all 745 rows instead of 720, adding `67242108` for the
2026-09-09 row and `ebe687dc` for the 24 from PR #185. The record's three Codacy
counts at checkpoint C4 are labelled for what each one measures — the live
subset awaiting F's service work (15 Sonar, 20 Codacy), the planned rows to send
(15 Sonar, 23 Codacy), and the calls F actually made (10 Sonar, 27 Codacy) — so
they can be reconciled instead of read as a contradiction. plan.md's inventory
path has its `# new:` marker back. The scope amendment's dated note moved after
the active requirement it had been inserted into the middle of, which is the
convention the audit itself sets. The audit's claim to be a "full inspection
list" is narrowed to the completion statements it actually tabulates.

One review finding was declined: the five `history` entries share their `why`
prose, but each row's `id` column identifies which finding it belongs to, which
is why the entry shape does not repeat it.

Gates, each read from its own exit code: 236 record CLI tests pass with 0
failures, ESLint clean, lizard reports no threshold exceeded, markdownlint
reports 0 issues in 15 files, `--check --planned` exits 0 with `745 ids: 0
missing, 0 duplicate, 0 open, 1 unconfirmed`, the record regenerates
byte-identically, and `git diff --check` is clean.
