doc: add CTC meeting minutes 2015-10-21 #3480

Trying out the name "CTC" (Core Technical Committee) for this meeting now, whaddy'all think? Would you prefer just "TC" with "Core TC" where possible?

Once we land on a name we have a few references to update: README.md, GOVERNANCE.md, directory name for doc/tsc-meetings/.

Context: The "TSC" is moving to a foundation umbrella role, to oversee the technical aspects all the Foundation's projects, this group is becoming the Core TC and is focused on core only, which has been what it does mostly anyway. Initially membership of both groups will be the same, that'll change over time with additions and resignations. See nodejs/TSC#2.

I read "CTC" in my head as "CDC", which for some reason I associate most closely with The Walking Dead, and I'm cool with that.
