Summary

• Keep top-level enabled as the plugin master switch.
• Keep provider-level enabled as the fetch control.
• Add provider-level showSidebarBar and showFooterBar visibility controls.
• Add typed provider-level sidebarWindow and ooterWindow selection.
• Preserve legacy 5h matching as
  olling and automatic footer fallback.
• Update schema, docs, examples, tests, and minor changesets.
