Preserve macOS sessions after refresh rejection or Keychain read failure - #25759
Supersedes #25665 and carries its auth-session persistence fix onto current main.
Classify refresh-token 400/401 responses: a rejection naming the refresh token (REFRESH_TOKEN_NOT_FOUND_OR_EXPIRED, REFRESH_TOKEN_ERROR, PROVIDER_REJECTED, invalid_grant) or naming no error clears the session; a named client rejection or a non-JSON page keeps it, with a cancellation-aware cooldown.
Reset refresh cooldown state when the active refresh-token session changes.
Treat only temporary Keychain read failures (locked or busy: errSecInteractionNotAllowed, errSecNotAvailable, on the current or legacy account) as unavailable during restore, preserving the cached session. A permanent failure reads as an empty store, so a session with no usable token still routes to sign-in after the file fallback is checked.
Remove unrelated test-registry entries and keep the auth regression tests deterministic.
The original PR branch rejects force updates after the required rebase, so this follow-up contains the rebased, review-complete head.
Remote controller build and hosted auth-refresh/package tests are being run for this exact head.
Changelog
Fixed: preserve signed-in macOS sessions when refresh requests or Keychain reads fail for reasons unrelated to the session.

Summary by CodeRabbit
Bug Fixes
Refresh requests rejected for request or client errors no longer automatically end the session; saved tokens are retained, and repeat attempts are briefly paused.
Errors that indicate an expired or invalid session still clear the session.
When token storage is temporarily unavailable, the app preserves the cached authenticated session instead of clearing it.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
Refresh handling now distinguishes session-ending responses from other refresh request rejections and applies a cooldown to the latter. Token storage availability now reflects Keychain read status and is checked again during session priming.
HTTP 400 and 401 refresh responses are classified by response content. The coordinator preserves the refresh token for request rejections. Tests cover responses that retain or clear the session.
Rejected-request cooldown and tests vendor/stack-auth-swift-sdk-prerelease/Sources/StackAuth/APIClient.swift, vendor/stack-auth-swift-sdk-prerelease/Tests/StackAuthTests/RefreshRejectionTests.swift, vendor/stack-auth-swift-sdk-prerelease/Tests/StackAuthTests/RefreshTestClock.swift, vendor/stack-auth-swift-sdk-prerelease/Tests/StackAuthTests/RefreshTransportFixture.swift, .github/workflows/auth-refresh-tests.yml
Rejected requests use a cancellation-aware cooldown. Cooldown state resets after success, definitive rejection, or refresh-token changes. Tests cover cooldown timing, session replacement, and delayed responses; the workflow includes the rejection tests in its filter.

Keychain reads track whether their results are definitive. The store adds a readability probe, and tests cover status classification.

The fallback store reports availability based on fallback use and Keychain read status. Session priming rechecks availability after token reads. Tests cover fallback availability and retaining the cached session when storage reports unavailable.
A Keychain read error on the refresh token, combined with a transient server failure at launch, can wipe a stored macOS session that the PR is meant to preserve. Handle read failures separately from an absent token during cached-session validation before merging. A test helper can also release waiters early, which is a minor flakiness risk.

Full details: Cmux Cloud Persistent Session And Early Input
Explanation
PASS. The authoritative diff changes authentication refresh classification, Keychain storage availability, session priming, and related tests/workflow wiring. It does not change Cloud terminal creation, cmux-tui clients, manual renderers, Ghostty runtime admission, PTY readiness, input ownership, or Cloud transport fences. Therefore no explicit failure condition in the cloud persistent-session rule applies.
Full details: Cmux Swift Actor Isolation
Explanation
No actor-isolation failure is introduced. The changed token stores are public actor types, and the changed SDK clients are actor-isolated. AuthCoordinator and MacAuthComposition remain explicitly @MainActor; the new composition closure crosses to the token-store actor with await. No changed service protocol, shared mutable Sendable reference type, or background access to a UI-bound store appears. The existing non-Sendable RefreshOutcome cross-actor plumbing predates this diff and is not materially worsened by adding the rejection case.
Full details: Cmux Swift Blocking Runtime
Explanation
No blocking primitive was added to production Swift. The new rejected-refresh cooldown uses the existing injected TokenRefreshClock.sleepUntil timer abstraction with cancellation and actor-owned state; it does not add direct Task.sleep, semaphore waits, polling, dispatch delays, or manual locks. The NSLock and virtual sleeps are confined to changed test scaffolding.
Full details: Cmux Browser Automation Off-Main
Explanation
The pull request does not change browser socket automation. The authoritative diff changes auth, Keychain, refresh-test, and workflow files only. It does not modify either rule-scoped file, Sources/TerminalController.swift or ControlCommandExecutionPolicy.swift, and no added or removed lines contain browser automation routing or WebKit/AppKit wait patterns. The check is therefore not applicable.
Full details: Cmux Expensive Synchronous Load
Explanation
PASS. The reviewed diff changes authentication refresh classification, Keychain token reads, session priming, and related tests. It adds no agent-history loader, transcript/trajectory/workstream JSONL handling, broad directory scan, or per-record agent syscall. The new Keychain probes run in actor-backed token stores, and refresh-body JSON parsing handles bounded HTTP error responses in APIClient; neither is an interactive main-actor load. No changed production Swift path references RestorableAgentSessionIndex, SharedLiveAgentIndex, or the prohibited agent files.
Full details: Cmux No Hacky Sleeps
Explanation
PASS: The pull request changes Swift production and test code only. The sole non-Swift change adds RefreshRejectionTests to a GitHub Actions test filter, and the rule explicitly excludes workflow YAML. No covered TypeScript, JavaScript, shell, or build/runtime-script delay was introduced.
Full details: Cmux Algorithmic Complexity
Explanation
PASS. The production diff adds no nested scan or per-target rescan over user-sized collections. The refresh classifier performs one linear Data.contains(where:) pass over the response body and constant-time Set.contains checks. Keychain and fallback changes perform a fixed number of sequential reads. No sorting, filtering, batch lookup, in-memory join, or unbenchmarked algorithm for workspace-scale records was introduced.
Full details: Cmux Swift Concurrency
Explanation
PASS. The cmux-owned Swift changes use async/await and actor methods only; they add no DispatchQueue, Combine, completion-handler, or fire-and-forget Task pattern. The only runtime Task is in the vendored StackAuth SDK, where it is stored in refreshRejectedCooldownTask, cancelled on reset/replacement, and tied to the cooldown lifecycle. Added test Tasks and continuations are test synchronization.
Full details: Cmux Swift `@Concurrent`
Explanation
No changed Swift code violates the annotation rule. The new storage APIs are actor-isolated methods on FallbackTokenStore and KeychainStackTokenStore, and macOS calls them through an awaited actor boundary from the @MainActor composition. The refresh changes remain inside the APIClient actor; the new cooldown task only awaits a clock and updates actor state. Test helpers add synchronization and virtual-clock waits, not CPU-, file-, or network-heavy UI work. The diff adds no @concurrent annotations and no invalid synchronous or actor-isolated use.
Full details: Cmux Swift Package Boundaries
Explanation
The production auth logic changed by this PR is already behind SwiftPM boundaries in Packages/Shared/CmuxAuthRuntime/Sources/CmuxAuthRuntime and in the vendored SDK. The only app-target change is a closure passed from Sources/Auth/MacAuthComposition.swift into AuthCoordinator, which is app-lifecycle composition glue. No new independently testable domain logic was added to the app target root, and vendored code is explicitly allowed by the rule.
Full details: Cmux Swiftpm Lockfiles
Explanation
No lockfile-policy violation is introduced. Packages/Shared/CmuxAuthRuntime/Package.swift is unchanged, so its existing package-local Package.resolved needs no update. No Xcode project or package-reference file changed, and the root Xcode Package.resolved is therefore not required. The workflow change only adds a test filter. Changes under vendor/stack-auth-swift-sdk-prerelease are vendored third-party source and test changes, which the rule exempts.
Full details: Cmux Swift Logging
Explanation
The diff adds one diagnostic call, AuthDebugLog.log for a Keychain probe. AuthDebugLog writes to unified logging and only writes its protected debug file under #if DEBUG && os(macOS). The message contains a Keychain status and account identifier, not a token or personal data. No print, debugPrint, dump, NSLog, new file-scoped Logger, or stdout/stderr logging is added.
Full details: Cmux User-Facing Error Privacy
Explanation
The production diff changes refresh classification, cooldown state, and Keychain-read handling. It does not add or materially change user-facing error text, alerts, command output, API error bodies, or recovery copy. The changed provider/error identifiers are internal classification values, and no concrete path to a cmux end user was found.
Full details: Cmux Full Internationalization
Explanation
The PR changes auth storage and refresh classification only. The changed production Swift adds no user-facing UI, alert, menu, error, or recovery text; added literals are protocol/config tokens or debug/diagnostic logs. No localization catalogs, web message files, metadata, changelog, or rendered markdown files are changed. Test and workflow changes are allowed by the rule.
Full details: Cmux Swiftui State Layout
Explanation
PASS: The pull request changes auth runtime, token stores, composition, and tests. No changed file imports SwiftUI or adds SwiftUI views, ObservableObject/@published state, GeometryReader, lazy/list row stores, or render-time state mutation. The check is not applicable.
Full details: Cmux Architecture Rethink
Explanation
PASS. The Swift changes keep ownership in the existing actors and coordinator. KeychainStackTokenStore owns read-status state, FallbackTokenStore consumes it, and APIClient owns refresh cooldown state. The new sleepUntil task implements the required cancellation-aware rejection cooldown, not a lifecycle or shared-state race workaround. The only new NSLock usage is in test fixtures, which the rule allows. The macOS composition adds one closure to the shared AuthCoordinator; it does not add duplicate UI lifecycle wiring or a new AppKit owner. Comments and tests state the key invariants for temporary Keychain failures, definitive rejection, and refresh-session changes.
Full details: Cmux Swift Auxiliary Window Close Shortcuts
Explanation
PASS. The PR changes authentication, token storage, SDK refresh handling, tests, and CI wiring. The reviewed Swift diff adds no NSWindow, NSPanel, NSWindowController, SwiftUI Window, or WindowGroup code. It also adds no standalone cmux window or auxiliary-window identifier requiring close-shortcut ownership. The auxiliary-window rule is not applicable.
Full details: Cmux Source Artifacts
Explanation
The diff changes only workflow configuration, hand-written Swift source, and auth regression/test fixtures. The new RefreshRejectionTests.swift is a deliberate test-system fixture, and the existing vendor/stack-auth-swift-sdk-prerelease checkout was already tracked at the base revision. No logs, screenshots, recordings, temp or cache directories, build output, DerivedData, package downloads, or broad scratch directories appear in the 13 changed paths.
Full details: Cmux No Test Or Debug Seam In Production Source
Explanation
The changed production Swift files add no #if DEBUG or other test-build-only seam, and no member uses the prohibited debug/test names. KeychainStackTokenStore.isReadable(), wasLastReadDefinitive(), and resetLastReadStatus() are public, but they are called by production FallbackTokenStore code; isTokenStorageAvailable() is called by production MacAuthComposition and session-restore code. The test-only status check remains non-public and is accessed through @testable import CmuxAuthRuntime. The changed #if canImport(Security) blocks are platform guards for real Keychain behavior.
Full details: Cmux Cache Substitution Correctness
Explanation
The pull request still substitutes cachedRefreshToken for an authoritative Keychain read in KeychainStackTokenStore.isReadable() (line 86). FallbackTokenStore.isTokenStorageAvailable() calls this path when no token read is pending, and MacAuthComposition uses it during session restore. The cold path probes Keychain, but a warm cache returns true without checking whether the Keychain item was removed or changed. No freshness, invalidation, or event-driven mechanism handles that stale-cache case. The added tests cover status classification and file fallback, but do not warm the cache and then change or remove the Keychain item.
Resolution
Do not return true from isReadable() solely because cachedRefreshToken is non-nil. Always perform the authoritative Keychain probe, or add an event-driven/versioned freshness mechanism that invalidates the cache when the Keychain source changes. Add a regression test that warms the cache, changes or removes the Keychain item, and verifies that session-restore availability follows the authoritative result.

The job is currently stuck in post-run checkout cleanup on the same low-space runner, so GitHub has not published a final conclusion. Regression for #25628. Fails on
the base: any 400/401 from the token endpoint deletes the refresh token.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…dable Keychain
The Stack SDK treated every 400/401 from the refresh endpoint as a dead
refresh token and deleted it. Stack answers the same statuses for a
rejected client (publishable key, OAuth client, request shape), which
says nothing about the session. Clear only when the answer names the
refresh token (REFRESH_TOKEN_NOT_FOUND_OR_EXPIRED, PROVIDER_REJECTED,
REFRESH_TOKEN_ERROR, OAuth invalid_grant) or names nothing at all.
On macOS a Keychain read that failed (not just found nothing) returned
nil and launch restore signed the user out. The Mac composition now
passes a Keychain readability probe as isTokenStorageAvailable, so an
unreadable Keychain preserves the cached session.
Fixes#25628
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…ges, clear on permanent Keychain errors
Review follow-up for #25628:
- A 400/401 refresh refused for a reason other than the refresh token
now starts the same cooldown as a 429 instead of retrying on every read.
- A 400/401 whose body is not JSON (a captive portal or proxy page) is
not Stack's verdict and keeps the session.
- Only errSecInteractionNotAllowed and errSecNotAvailable count as an
unreadable Keychain; a permanent read failure still routes to sign-in.

Thanks for flagging this. I checked the current classic base: the shared AuthCoordinator availability hook is wired by MobileAuthComposition, and there is no separate CloudAuth composition in the classic graph changed by this PR. I’m keeping this classic PR scoped; if a feat-cmux-next CloudAuth composition omits the hook, that belongs in a follow-up on that branch.
…ed rejections
Only a locked or busy Keychain (errSecInteractionNotAllowed,
errSecNotAvailable) now counts as unreadable at restore. A failure that
never clears read as unavailable forever, keeping a signed-in shell with
no usable token. A 400/401 refresh rejection that names no error again
ends the session; only a non-JSON page or a named client rejection keeps
it.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
High, fixed: a permanent Keychain failure kept a dead session forever. Every status other than success or not-found counted as "unavailable". That included errSecAuthFailed, errSecDecode and errSecMissingEntitlement, which never clear, so restore preserved a signed-in shell with no usable token on every launch. Now only errSecInteractionNotAllowed and errSecNotAvailable are temporary. Any other failure reads as an empty store, and FallbackTokenStore checks the file fallback before sign-in. onlyTemporaryReadFailuresCountAsUnreadable covers this.
Medium, fixed by 1: found tokens skipped validation after a non-definitive legacy read. With permanent statuses now definitive, only a locked or busy Keychain reaches the preserve branch, and that is the case where preserving is correct.
Medium, fixed: an empty or no-error 400/401 kept the session. This contradicted the original fix (Fixes #25628 #25665): a dead token answered without a code would retry forever. An empty body or JSON naming no error is now sessionEnded again. A non-JSON page and named client rejections still keep the session. unnamedRejectionClearsTheSession (3 bodies) and RefreshDeadlineTests (401 clears) cover this. The late-rejection epoch test now sends a named client rejection, so it still exercises the cooldown.
Not changed (low):
A sticky lastReadWasDefinitive can carry a failure from an unrelated read into the next restore. It only errs toward preserving, and after fix 1 only for a locked or busy Keychain. Returning status from the coordinator's own reads would be a wider API change, so it is left for a follow-up.
The store plumbing still has no SecItemCopyMatching seam test. Coordinator coverage uses a scripted probe.

Bind read status to the token probe.
A concurrent AuthCoordinator.accessToken() call performs its own storage-availability check before reading a token. That check can consume the failed FallbackTokenStore read status before session priming performs its post-read check. If the Keychain is readable on the later probe, priming can clear the cached identity.
Return availability with the token-read result, or bind the status to the same serialized probe. The correction belongs in FallbackTokenStore or KeychainStackTokenStore, not at cachedUser: fixtureUser.

- Line 61: Update FallbackTokenStore or KeychainStackTokenStore so token-read
availability is returned with the token result or its status remains bound to
the same serialized probe. Ensure a concurrent AuthCoordinator.accessToken()
availability check cannot consume the failed-read status before session priming
performs its post-read check, and leave cachedUser handling unchanged.

A 401 whose body is not valid UTF-8 (a proxy page in a legacy encoding)
is not a Stack rejection, but classify treats it like an empty body and
ends the session.

A 401 page that is not valid UTF-8 failed the String conversion and was
treated as empty, ending the session. Only an empty or whitespace body
now ends it; anything else that is not Stack JSON keeps the session.

No blocking findings. The commit matches its intent. The subagent checked that the test fixtures still exercise what they claim and that every caller compiles.
Low, fixed.RefreshRejection.classify treated a 400/401 body that is not valid UTF-8 as empty, which ended the session. For example, a proxy page in ISO-8859-1 hit this path, even though the code's own rule says a non-Stack page keeps the session.
b7f17d8 adds the failing regression RefreshRejectionTests.nonUTF8PageKeepsTheSession.
ed29243 decides "empty" on the raw bytes instead, so only an empty or whitespace-only body ends the session.
Low, accepted. A Keychain error that clears on its own but is not errSecInteractionNotAllowed or errSecNotAvailable now shows the user as signed out for that launch. Examples are errSecInternalComponent, or errSecMissingEntitlement while securityd restarts.
clearAuthState clears only the cached user and session, not the stored tokens, so the next launch restores the session.
Debug builds without the entitlement still fall back to the file store.
This is the intended trade-off against keeping a dead session forever.
Verification: fleet run-swift-testing-suites.sh runs of the SDK package on b7f17d8 (expected red) and ed29243 (expected green) are in progress, along with hosted CI on ed29243.

- Line 121: Update isDefinitiveReadStatus and the refresh-token validation flow
so a failed Keychain read is distinguishable from a genuinely absent refresh
token; in particular, classify errSecDecode appropriately during restore and
carry the read outcome into validateCachedSession. Preserve the cached session
when the refresh-token read failed, and only clear persisted session state when
the token is confirmed absent.

…llow-up
Brings the branch to main ebc53e71965 and takes the review fixes made on
#25759 (head ed29243), which
was opened because this branch rejects force pushes:
- Refresh rejection classification is private to APIClient; rejected
requests cool down on a cancellation-aware timer that resets when the
refresh-token session changes.
- Keychain availability probes the legacy refresh-token account, and a
permanent Keychain failure or an unnamed rejection ends a dead session.
- A non-UTF-8 proxy page keeps the session; an empty body is decided on
its raw bytes.

Summary
Supersedes #25665 and carries its auth-session persistence fix onto current
main.REFRESH_TOKEN_NOT_FOUND_OR_EXPIRED,REFRESH_TOKEN_ERROR,PROVIDER_REJECTED,invalid_grant) or naming no error clears the session; a named client rejection or a non-JSON page keeps it, with a cancellation-aware cooldown.errSecInteractionNotAllowed, errSecNotAvailable, on the current or legacy account) as unavailable during restore, preserving the cached session. A permanent failure reads as an empty store, so a session with no usable token still routes to sign-in after the file fallback is checked. The original PR branch rejects force updates after the required rebase, so this follow-up contains the rebased, review-complete head.

Fixed: preserve signed-in macOS sessions when refresh requests or Keychain reads fail for reasons unrelated to the session.
