fix(migrations): treat duplicate-column ALTER as no-op - #1886

Some upgrade paths leave a column already present from the initial CREATE TABLE statement, then a later migration tries ALTER TABLE ADD COLUMN — SQLite errors. Wrap each ALTER in try/catch and log "already exists, skipping" instead of failing.

Code Review
This pull request modifies the migration runner to handle 'duplicate column name' errors by manually marking migrations as applied in the database. A critical issue was identified where this approach bypasses the transactional integrity of the migration process; if a migration contains multiple statements and fails on a duplicate column, the entire transaction rolls back, but the migration is still recorded as successful. This leads to an inconsistent database state. It is recommended to utilize the existing idempotency mechanism to check for schema state before execution.

This implementation marks a migration as 'Applied' even if it partially failed and rolled back, which can lead to an inconsistent database state.
Because applyMigration() (defined at line 637) is wrapped in a transaction, any error—including a 'duplicate column name' error—triggers a full rollback of all statements within that migration file. If a .sql file contains multiple operations (e.g., adding a column and creating an index, or updating data), the subsequent operations will be rolled back and never executed, but the migration will be recorded as 'Applied'. This leaves the database in a state where some expected schema changes are missing, and the runner will skip this version in future runs.
Recommendation:
Instead of this global catch-all, use the existing idempotency mechanism. Add the specific migration version and the column it's checking for to the isSchemaAlreadyApplied function (starting at line 227). This allows the runner to detect the pre-existing state before starting the transaction, safely skipping the execution while still recording the migration as applied. This maintains the 'all-or-nothing' guarantee of the migration runner.

Some upgrade paths leave a column already present from the initial CREATE TABLE statement, then a later migration tries ALTER TABLE ADD COLUMN — SQLite errors. Wrap each ALTER in try/catch and log "already exists, skipping" instead of failing.
