docs(coc): Turn the code of conduct email into a form that a group of people will get

In #887, we noticed the code of conduct has an email that is likely not part of our team. I suggest that we consider using a google form that goes to a general email (google group maybe?) that a few of us get. I don't anticipate us getting lots of emails but if we have a form, then we can easily monitor a private google group and add more people to it in the future if we need to.

This might be another pr - but i wonder who owns this email? is this really the one we should be using in this repo?

Something we aim for in @the-turing-way is to have a CoC contact who is outside of the project. That way, if someone has a problem with one of the project leaders (or someone working on the CoC) they will have more confidence that the process will be confidential and fair. It would be good to reach out to some of our communities to see if anyone would be willing to play that role.

I love that @JimMadge, why don't we leave this issue open and add a "help-wanted" label to it? I love the idea of having an outside team. Perhaps someone will be interested in working on this part of the project, as it offers a beginner-friendly approach for others to get involved.

I did not yet test out that email. I am a bit nervous to cold email that one as it looks spammy to me!!

So update on this: I just went to add Felipe to this repo and noticed that kentdodd is actually listed as a maintainer with write access.

@kentcdodds in case you see this, we are trying to figure out if the email in our code of conduct is still good. It seems to be an email that belongs to you / your domain! At first, because I'm new here, I thought it was not a real email or maybe someone copied the COC here from your site. Now I see you were a part of this project at some point!! 🙌🏻 So any input on the COC and whether it's ok for emails to go your way or not is helpful!! I don't expect a lot of emails but you never know when something might come up.
