feat(router): make the two-tier cost function the default KV policy - #17776
Makes the two-tier cost function (dynamo-two-tier-cost-fn) the default worker-selection policy for --router-mode kv. Closes DYN-5308.
Important
Depends on #17792 (announcement, factory, and test fix), whose two commits are included here until it merges. Merge this only after the release/1.7.0 branch is cut, so the swap ships in 1.8 as the notice says.
lib/llm/src/kv_router/plugins.rs: the installed registry and the fallback registry use two_tier_default_factory() instead of the current default.
lib/llm/src/entrypoint/input/http.rs: the startup notice switches to present tense ("defaults to", "the previous cost function").
Docs: the router configuration page describes two-tier as the default, default as resolving to it, and dynamo-default-cost-fn as the pre-1.8 default.
Tests:
selection_matches_frozen_frontend_trace pins dynamo_custom_policy_builtin::default_factory(), so the frozen trace keeps covering the additive cost function.
kv_chooser_ignores_stale_presence expects the default pool to report no runtime parameters.
To opt out, configure an instance of the previous cost function:
Cost-function flags have no effect. overlap_score_weight/overlap_score_credit (except 0, which turns off the cache tier), router_temperature, prefill_load_scale, and the host, disk, and shared cache weights all tune the previous cost function. Two-tier reads only its three thresholds.
PATCH /router_policy returns 422 for default pools, because two-tier has no runtime parameters. GET /router_policy lists them with configuration: null.
Preferred taints are not applied. The previous default multiplied each worker's cost by the preferred-taint factor from disagg topology preferred enforcement and request routing constraints; two-tier does not read it, so that soft same-domain bias is gone under the default. Deployments that rely on it can select dynamo-default-cost-fn. Follow-up: DYN-5312.
The load signal is the active request count, not prefill tokens or decode blocks. This also applies to plain decode pools. Ties go to the first candidate row, where the previous default sampled randomly among ties.

Walkthrough
The KV router now uses a two-tier cost function as its default policy. The picker can select workers using load alone or cache and load inputs. The HTTP frontend warns when no custom policy is configured, and the guide documents the default and compatibility option.

Summary
Define the two-tier default policy lib/router-plugins/builtin/src/two_tier_cost_fn.rs, lib/router-plugins/builtin/src/lib.rs
The picker supports load-only selection and the existing cache-and-load path. The new default factory configures cache reads based on worker type and policy parameters, and enables exclusive session affinity. The factory is publicly exported.
Install the default policy lib/llm/src/kv_router/plugins.rs, lib/llm/src/kv_router.rs, lib/llm/src/discovery/model_manager.rs
The router registry attaches the two-tier default factory. Tests update their expectations and use the additive cost model where specified.
Document and report the default lib/llm/src/entrypoint/input/http.rs, docs/fern/pages/developer-guide/knowledge-base/modular-components/router/configuration-and-tuning.md
The HTTP frontend warns when KV routing has no custom worker-selection policy. The guide documents the two-tier default and how to select the previous cost function.
Making the two-tier policy the default changes routing for existing KV-mode deployments. Deployments that rely on preferred topology routing lose that bias without any configuration change. The runtime-update docs now describe controls the new default does not support, and the opt-out warning can be skipped when only some stages use a custom policy. Resolve the preferred-taint behavior and fix the docs before merging, or explicitly accept and document the regression.

Correct the runtime-update section for the new default.
The YAML example explicitly selects dynamo-default-cost-fn. This is correct when users want the previous additive cost model and its runtime controls; it does not rely on the new default.
The runtime-update section incorrectly says that the built-in default supports the additive scoring parameters and PATCH /router_policy. The built-in dynamo-two-tier-cost-fn does not support runtime configuration, so PATCH returns 422 and GET reports configuration: null. State this limitation and identify dynamo-default-cost-fn as the policy to select when runtime scoring updates are required.

Add the factory hosts will install to make dynamo-two-tier-cost-fn the
default worker-selection policy. It keeps session-affinity targets exclusive
and, on plain decode pools and with zero overlap credit (the load-aware
preset), declares load-only inputs so those routers still run no KV indexer.
Hosts still install the current default.
The HTTP frontend logs a startup notice when --router-mode kv runs without a
configured worker-selection policy, and the router configuration docs
announce the 1.8 change and the dynamo-default-cost-fn opt-out.

The test's frontends subscribe to KV events over NATS, but the mockers
published on the runtime default (ZMQ), so the routers never saw the prefix
blocks. The test still passed because the previous default broke equal-cost
ties at random; the two-tier default breaks them by row order and failed
every attempt.

Hosts now install dynamo-two-tier-cost-fn as the default worker-selection
policy for --router-mode kv. The startup notice and router docs now describe
the default as current.
The frozen frontend selection trace pins the previous cost function so it
keeps covering it, and the stale-presence test expects the default pool to
report no runtime parameters.

Closing until the 1.8 cycle. #17792 lands the startup notice and a single host_default_factory() seam, so the swap will be a one-line change in a new PR after release/1.7.0 is cut. The policy choice for that swap (request-count two-tier vs. a token-aware variant) is still open; preferred-taint support is tracked in DYN-5312.
