ci: Improve release changelog by adding the PR to items as well as the username of the contributor

This is actually a "fork" from @changesets/changelog-github with some key changes to be more suitable for us.

There are two main changes:

• Adds the PR number to the change
• Update dependencies are groups and collapsed by default
