feat(OMN-7836): add model names to delegation charts + fix quality gate pass rate

Summary

• Extends DelegationSummary with byModel breakdown and qualityGatePassed/qualityGateTotal fields
• Both 2D and 3D delegation chart variants now render model names alongside task type breakdown
• Quality gate pass rate now shows X / Y passed detail for transparency into the raw counts
• Storybook fixtures updated with model data (Qwen3-Coder-30B, glm-4-plus, codex-cli, gemini-cli)
• Widget version bumped to 2.1.0

• Delegation charts show actual model names (not "build-loop-orchestrator")
• Quality gate pass rate reflects pass/total detail for transparency
