Implement the full Application and Infrastructure layers: - CQRS commands: Create, Update, Delete, Confirm, Complete, Cancel, MarkNoShow - CQRS queries: GetById, List (paginated, filtered, sorted) - EF Core configuration with snake_case tables, owned CalendarSyncInfo - Repositories (write + read with tenant filtering) - AppointmentsController with 10 REST endpoints - FluentValidation for commands - 33 new unit tests (handlers + controller) - All 201 tests passing Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
