test(api): lock chat/Completions OpenAI metadata honesty over HTTP

• Re-ship tip substrate (server.py, orchestrator.py, cost_ledger.py) plus fixture model-field alignment from bare main.
• Restore real HTTP outcome tests for OpenAI metadata shape honesty on chat Completions / Completions.
• Cumulative tip honesty tests (tool_call_id, unknown fields, Responses instructions/reasoning) included.

Test plan

• python -m pytest tests -q — 325 passed locally
• tests/test_chat_openai_metadata_http_honesty.py (8 cases)
• CI Full unit + Semgrep product gates
