Add issue template for different puppet

Currently puppet-service supports multiple service providers, but issues related to different puppets are not clearly managed. So we need to create rules for managing issues related to different types of puppet provider.

Here is my thoughts:

1. puppet-service is the perfect repo to manage all types of puppet service tokens, so we could use this repo holding all issues related to all puppet-service tokens.
2. We should create multiple issue template for different puppet provider, these templates will automatically attached corresponding labels to those issues, then we can easily separate issues to different providers.
3. Create github teams for different providers, and put these teams as the default assignee for each template.

I believe with these templates, developers could be easier to get help from the right person.
