[ROCm] fix get_device_name for rocm

Use amd-smi to get the device name for AMD GPUs.

• This fixes the hard-coded "AMD" name introduced in [ROCm][V1] Add intial ROCm support to V1 #12790
• Proper device name is required to distinguish triton's tuned configs from each other for AMD GPUs. e.g. MI300 vs MI325

Tested using:

VLLM_ENABLE_V1_MULTIPROCESSING=0 VLLM_USE_V1=1 python benchmark_latency.py --model Mixtral-8x7B-v0.1/ --load-format dummy -tp 8
