OSA: Review fixes for toolsets, skills, and MCP

This is a PR that handles feedback on the other PRs in this stack, instead of modifying those PRs directly. It's based on review comments on those PRs.

What I did

• Changed toolset API slightly to match Open Services more closely: schema/outputSchema → input/output
• renamed consumer to transport everywhere to match our terminology.
• getRef → getToolName because ref has a different meaning in SB (composition). see Skills M2b: Rework the core toolsets into their intended shape #35726 (comment)
• Removed static MCP tool name conversions, as I don't believe tool name changes are a breaking change, but we'll see if we get complaints about this, and I have to back track.
• Removed telemetryGroup as it was overly specific to the old MCP tool setup, and didn't make sense to be in the fundamental toolset API (which group would community addons use?). The migrated tools can just have the toolset group in their telemetry payload as before.
• Moved the whole test toolset into addon-vitest, as core shouldn't try to be responsible for that.
• Replaced the explicit "allowlist" on the module graph with lazily starting it instead on first query, meaning that the module graph now initiated when a tool uses it, and not eagerly in pre-determined situations. In the end this is more flexible and practical while having the same performance outcome.
• Change detection (the git scan) starts separately from the graph: stories.findByComponent only hosts the graph, stories.changed also starts the scan. Help/docs/test-run pay for neither.
• Fail stories.changed when the module graph or change detection isn't actually ready, instead of returning an empty change set that looks like "nothing changed".
• remove the uiRoot from context, changed origin to include the optional subpath for UI instead, as there was no use case for having the subpath-less context too.
• Join MCP endpoint URLs with the Storybook origin's subpath, so a Storybook hosted at /storybook/ doesn't drop that prefix when the CLI proxies to MCP.
• Reject malformed or colliding derived MCP tool names at registration, so a bad id fails loudly instead of silently overwriting another tool.

3. Run yarn storybook tools --help and yarn storybook tools docs list against that config; confirm they complete without waiting on the module graph (help/docs never host it).
4. Run yarn storybook tools stories find-by-component --componentPaths '["<some-component-path>"]' and confirm it returns story ids (this is the first command that hosts the graph).
5. Run yarn storybook tools stories changed and confirm it either lists changes or fails with a graph/change-detection reason, not a silent empty "no changes".
6. Run yarn storybook tools test run --stories <some-story-id> and confirm it completes.
7. Run yarn storybook skills get stories and confirm instruction text uses CLI transport command spellings (npx storybook tools …), not the old consumer wording.
