[vivado_ip] constrain minimal size of variable-sized output ports #980

This is a follow-up of #978 and #976 to fix some Vivado IP issues.

Vivado has issues handling zero-sized ports. Hence, this PR constrains ALL variable-sized ports to a minimal size of 1. Dedicated enable switches (like IO_PWM_EN) allow to remove modules that are not required.
