fix(middleware): make the admission queue bound actually bind (FIFO handoff, no wake-all) - #2265
Conversation
--queue-size never bound the number of queued requests: the queue processor drained the size-capped channel and spawned a detached waiter task per request, so every overflow request waited anyway (production read smg_admission_queue_depth=1288 with --queue-size 512), and notify_waiters woke the whole herd to race for each returned token. Replace the channel-plus-processor design with a slot semaphore held for the duration of the wait: at most queue_size requests park, the next arrival sheds 429 admission_queue_full immediately, and a waiter that outlives queue_timeout_secs sheds 503 admission_queue_timeout. Waiters park directly on the token bucket, which now hands returned tokens to the oldest waiter when refill_rate=0: FIFO order, no lost wakeups, no barging past the queue, and cancelled or timed-out waiters return any uncollected grant. Shed bodies, Retry-After, the permit-rides-response-body lifecycle, and the admission metrics are unchanged; smg_admission_queue_depth can no longer exceed queue_size. Clean, well-designed replacement of the unbounded mpsc+processor design. The semaphore-based queue bound is structural, FIFO grant handoff via per-waiter Notify is correct (stored-permit semantics close the lost-wakeup window), and FifoWaiterGuard handles cancellation/timeout cleanup for both granted-but-uncollected and still-queued states. No barging possible since try_acquire checks for parked waiters. The refill-rate>0 path is unaffected (new calls are no-ops when the waiter deque is empty). Test coverage is thorough.

Both helpers spin without a deadline. If a future change stops releasing a slot or stops enqueuing a waiter, the test blocks until the CI job times out, and the failure gives no diagnostic. Add a deadline with an explicit assertion message.

Description
Problem
--queue-size 512does not bound the admission queue. In production we measuredsmg_admission_queue_depth= 1,288 with a configured 512. The size cap only applied to an internal mpsc channel; a processor task drained that channel and spawned a detached waiter task per overflow request, so the real waiter population was unbounded. On top of that, every returned permit woke the whole herd (notify_waiters) to race for one token, waiters re-polled on a 10ms fallback loop, and a real lost-wakeup window existed between a failed try_acquire and parking.Solution
Two structural changes, both smaller than patching the old design:
Semaphore(queue_size): a request that misses the fast path must take a queue slot or is immediately shed 429. The depth gauge lives inside the slot's lifetime, so depth can never exceed the configured size - the bound is structural, not advisory.VecDeque, per-waiternotify_onewith the grant recorded under the bucket mutex). No herd wakeups, no polling loop, no barging past parked waiters, and the stored-permit semantics ofNotifyclose the lost-wakeup window. A cancelled or timed-out waiter dequeues itself and returns any uncollected grant.Preserved exactly: 429
admission_queue_full/ 503admission_queue_timeoutwithRetry-After: 2, the permit-rides-response-body lifecycle, all three admission metric names/labels, and timeout counted from queue entry. The refill-rate>0 (RPS mode) path is unchanged.Changes
