sgl-project#34329 picks the replay layout with is_dsv4_hisparse, which is also true for unified-KV whose C4 rows are linear bf16, so share one is_dsv4_paged_layout predicate with the swap-in selection. Also drop the two fused-compress env knobs that sgl-project#34926 deleted.

This is to replace the prior PR enabling prefetching for HiSparse: #29637.
