Rename

• srt/layers/fused_moe -> srt/layers/fused_moe_grok
• srt/layers/triton_fused_moe -> srt/layers/fused_moe_triton

Later, we will probably have srt/layers/fused_moe_turbomind. Then, all these files will be listed adjacently in alphabetical order.
