Handle empty input in deduplicate-kp2023. If no unmatched records are found by match-kp2023, do not write to the unmatched manifest.

Add newly encountered ethnicity values to ethnicity mapper.
For KP2023-specific functions that create ICD-10 and symptom condition resources, use the create_codeable_concept function to create the code entry, so that these condition resources can be correctly parsed by the fhir etl job.
