Specify the partner-user's name in addition to their email address upon creation

Summary

When you add a user to a partner you are prompted to enter an email address. We should also prompt for the user's name.

• The pop-up for creating a new partner user has a second field, name
• The name is saved for the user
• Specs to validate that the new field works
