feat(codex): add PreToolUse hook support

• add Codex CLI PreToolUse hook processing via rtk hook codex
• patch Codex hooks.json during rtk init --codex --auto-patch
• update Codex hook documentation and support matrix

Closing in favour of #3552, which covers the same Codex PreToolUse integration, is the version a collaborator has reviewed, and uses the transparent updatedInput path that Codex now applies end to end.
